Bayshore’s Integrated Care Solutions is seeking a Manager of Clinical Practice to lead our Bundled Care Program, supporting clients and caregivers through high-quality, community-based care. In this leadership role, you’ll collaborate with interdisciplinary teams, mentor frontline staff, and ensure care delivery meets the highest standards of excellence, compassion, and reliability.
This position requires full-time, in-person presence five days a week at our Markham office.
What You’ll Do
- Provide fill-in support across multiple programs and serve as the go-to resource for backup coverage, ensuring continuity of care and team support during absences or high-demand periods.
- Lead holistic care delivery with nursing, PSWs, allied health professionals, and social workers.
Partner with hospitals, HCCSS, and community organizations to ensure safe transitions.
