At Humber River Health we continue to advance the important role we play as we advocate for healthcare initiatives in the diverse community we serve.
The RCC Church Campus brings five hospitals under one roof to provide specialized care for patients who no longer require acute care, and are waiting in hospital for transfer to an appropriate alternate discharge destination, such as convalescent, slow stream rehabilitation, or long-term care.
Each of the hospitals, as an extension to its main hospital site, staffs and operates its own unit(s). Under the Post Acute and RCC Program, Humber River Health operates one 34-bed patient care unit.
This program demonstrates our focus and commitment to comprehensive care of the older adult and provides an exciting opportunity for physicians interested in working with this patient population.
Position Summary
As a member of the Department of Medicine or Department of Family and Community Medicine, you will help provide comprehensive medical care from admission to discharge to patients at the RCC in the capacity of the Most Responsible Provider (MRP).
The position works in collaboration with Nurse Practitioners, nursing and members of the inter-professional health care team, including enhanced Allied Health resources to deliver comprehensive care and treatment plans, assessment and management of acute medical and aging conditions, preventing or delaying complications, and restoration of optimal functional ability through an enhanced activation program.
This position includes on-call responsibilities.
In addition to demonstrated clinical excellence and outstanding interpersonal skills, you require relevant certification from the Royal College of Physicians & Surgeons of Canada and are a member of the College of Physicians & Surgeons of Ontario.
HRH has an expectation that all physicians will apply for an academic appointment in the Temerty Faculty of Medicine, University of Toronto.

Qualifications, Skills and Attributes
The ideal candidate for this position will have experience in team-based care of older adults and possess strong interpersonal skills.
Additional training in Care of the Elderly is preferred.
