KeyResponsibilities :
- Conductthorough medication reviews and essments to identify potentialdrugrelatedproblems.
- Developand implement individualized medication therapy plans incollaboration with patients and healthcareproviders.
- Providepatient education on medication usage adherence and lifestylemodifications.
- Monitorpatient progress and adjust therapy based on clinicaloutcomes.
- Doentand report medicationrelated interventions andoutcomes.
Requirements
- Doctorof Pharmacy (Pharm.D.) degree with training in medication therapymanagement.
- Boardcertification in medication therapy management (BCMTMS orequivalent)preferred.
- Strongytical and clinicalss.
- Effectivecommunication and interpersonal ss for patient and providerinteractions
