Setting :
This position entails community-based work, involving 3-4 visits per day either at clients' homes or hospitals, depending on their location (Must have a car)
Area of Focus : Westend of Toronto (Bathurst St to the east, St Clair Avenue to the north, the Humber River to the west and Lake Ontario to the south)
Compensation : $40 per hour + Mileage covered (0.61 / km from client to client)
Start Date : ASAP
Duration : 6-Month + high possibility of extension
Tenure : Full-time
Shift : 9am-5pm
Job Summary :
The Nurse Case Manager serves on a multidisciplinary team, delivering client-centered care to individuals with severe and persistent mental illness.
Responsibilities include conducting assessments, administering medications, developing treatment plans, and providing health education.
This role requires a mix of clinical, advocacy, and case management skills, working closely with clients, families, and healthcare providers to ensure high-quality, holistic care.
Key Responsibilities :
- Client Care : Conducts mental and physical health assessments, medication management, and health education.
- Treatment & Case Management : Develops, implements, and evaluates client care plans, coordinating with team members and community resources.
- Client Advocacy : Ensures quality care, provides counseling, supports client goals, and connects clients to resources (e.
g., housing, financial aid, vocational training).
- Collaboration & Consultation : Works closely with hospitals, community providers, and other stakeholders to facilitate continuity of care.
- Documentation & Compliance : Maintains client records, complies with confidentiality and health regulations, and adheres to policies.
- Crisis Intervention : Provides crisis support and assistance with daily activities such as budgeting, shopping, and transportation.
Qualifications and Skills :
- Education : Registered Nurse with a valid Certificate of Competence from the College of Nurses of Ontario.
- Experience : Minimum 3 years of psychiatric nursing experience, preferably in a community setting.
- Skills : Strong clinical assessment, planning, and documentation skills; effective communication; conflict resolution;
proficiency in MS Office.
- Other Assets : Experience with dual disorders, legal issues, and substance abuse preferred. Additional language fluency is an asset.
- Requirements : Valid Ontario driver's license and regular access to a reliable vehicle. Criminal background check required.
Working Conditions :

- Hours : 37.5 hours per week with flexibility based on client needs; occasional special event attendance may be required.
