Position Summary
The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team the Primary Health Nurse II provides care to individuals promoting wellness prevention of disease and / or disability and the management of chronic diseases / conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links / referrals to other human services that would enable the client to achieve their health goals.
Qualifications
Education
Graduate of approved Registered Nursing education program BN preferred.
Must be a member in good standing with CRNM
Skills Expertise and Experience
A minimum of five (5) years experience working as a Registered Nurse
A minimum of three (3) years experience working in a community health setting
Knowledge and Experience working with Students and / or Volunteers
Current CPR Certification at Basic Rescuer Level

Proven knowledge and ability in critical thinking decision-making problem-solving and crisis management
