THE OPPORTUNITY

Job Summary

The Access Coordinator is responsible for providing point of careleadership to the admissions team supporting centralized intake processes forall bed-based substance use programs.

The Access Coordinator will also overseethe Program Navigators who support intake, transition, and discharge planningacross all bed-based programs.

Essential Duties andResponsibilities

Program Management

  • Coordinates the work of the IntakeSpecialists and Program Navigators on a day-to-day basis
  • Ensures intake staff followestablished criteria and prioritizes intakes based on criteria, leads, andfacilitates actions related to daily client flow, plans for client transfer ordischarge and appropriate client placement / referral
  • Liaises with the clinical careteam to ensure specific care plan actions and processes are completed andreviews the intake and discharge planning of clients to ensure optimalprogression to discharge, in collaboration with the other clinical team members
  • Responsible for ensuringadmissions team files and manages all verification paperwork as required toadhere to accreditation, licensing and safety guidelines, and / or fundingsources.
  • Acts as a point person forclients, families, and community care teams who have concerns or questionsregarding their care;

follows ups as required

Administration

  • Provides oversight related toclinical and organizational data collection across all MHSU programs; preparesreports for MHSU leadership and funders, as required.
  • Manages central client calendar toensure program participants are supported by program navigators to attendscheduled appointments offsite as part of their treatment

HumanResources

  • Assists the Manager by attendingpanel interviews, providing recommendations on the selection of new staff,providing feedback to the Manager on staff performance appraisals, and optimaluse of budgetary resources
  • Determines related training andorientation requirements
  • Assigning and adjusting workload,schedules, and coordinating staff when coverage is needed, ensuring servicelevels, and staffing requirements are met

Community Engagement

Participates in funder meetingsand community stakeholder committees on behalf of the program as required bythe MHSU leadership

THE CANDIDATE

Qualifications

  • Post-secondary diploma in socialwork, community services, etc., or an equivalent combination of education andexperience
  • 2to 4 years of practical experience in an admission setting working withvulnerable populations, substance use services, mental health, or corrections
  • Satisfactory completion of acriminal records check with vulnerable sector screening is required
  • Satisfactory completion ofTuberculosis Screening form is required
  • Satisfactory completion ofEmployee Immunization Record form is required
  • First aid certification with CPRis required
  • Validdriver’s license with driver’s abstract
  • Highlevel of integrity and dependability with a strong sense of urgency andresults-orientation
  • Demonstrated strong written,verbal and interpersonal communication skills
  • Abilityto lead and work in a trauma-informed practice
  • Demonstrated commitment to thesocial sector with a passion for the organization’s mission is essential
