Program Summary: The Path Home – Pine Villa is a short- term transitional housing program for hospital patients who no longer require acute care but still require care and support in preparation for a long-term living arrangement.

Pine Villa specializes in supporting older adults (55+) with complex physical health challenges, mental health challenges, addiction or cognitive impairment.

The Path Home – PIne Villa, provides client centered and goal-oriented support services that enable clients to continue their recovery in the community as they wait for permanent housing. The Path Home – Pine Villa is a unique partnership between LOFT Community Services, SPRINT Senior Care and Sunnybrook Life Sciences Centre.

It is funded by the Ontario Ministry of Health and Long Term Care.

This partnership reflects the transformation of health care in Ontario which brings the expertise and some of the resources of a hospital setting to a community based transitional housing program.

Some of these resources include on site clinical practitioners including an Occupational Therapist, Physiotherapist, Nurse Practitioner and Advanced Practice Nurse.

Job Summary: As the Registered Practical Nurse , you will be part of an interdisciplinary team, who is expected to ensure the safety and security of the resident environment, to practice in accordance with College of Nurses of Ontario professional standards, and the policies and procedures as defined by LOFT Community Services.

The RPN will be responsible for working with individuals from diverse backgrounds, contributing to Program development, participating in clinical supervision; and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment.

The RPN is directly responsible for developing and articulating strategies for the management of recovery and psychosocial rehabilitation using information collected through assessments and collaboration with the PSWs.

Additionally, you will liaise with community agencies, other practitioners, and key partners to coordinate care for clients.

What You Will Do: Provide direct nursing care and medication management, including safe medication administration, order transcription, medication ordering and documentation (MAR), secure medication storage, and compliance with College of Nurses standards, pharmacy protocols, and organizational policies.

Monitor and assess clients’ physical, mental, and behavioural health, reporting changes, concerns, and incidents as appropriate.

Support client care planning, goal setting, and successful transitions to long-term or discharge destinations.

Respond to behavioural, cognitive, and crisis situations, including conflict resolution and emergency intervention as needed.

Conduct assessments, maintain accurate documentation, and ensure required supplies and equipment are available.

Promote a safe, clean, and supportive environment while following infection prevention and control practices.

Collaborate with physicians, pharmacists, healthcare professionals, community partners, clients, and families to coordinate quality care.

Provide clinical guidance and support to staff on health-related matters.

Foster respectful, recovery-oriented relationships with clients while maintaining confidentiality and professional standards.

Participate in on-call rotations, team meetings, training, care conferences, and other organizational activities.

Support interdisciplinary team operations and perform additional duties as assigned, including backup support for Personal Support Workers.

Perform other related duties as assigned in support of program operations and client care.

What You Bring: Completion of a recognized RPN program with medication administration training/certification.

Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).

Experience supporting older adults and seniors with mental health, cognitive, addiction, and responsive behavioural challenges.

Valid First Aid and CPR certification, with a clear Criminal Record and Vulnerable Sector Check.

Knowledge of community resources, social services, and supports available to adults and seniors.

Strong understanding of recovery-oriented care, psychosocial rehabilitation, and therapeutic relationship principles.

Excellent assessment, problem-solving, counselling, communication, and interpersonal skills.

Ability to work independently and collaboratively within an interdisciplinary team environment.

Proficient with MS Office applications and clinical documentation.

Physically capable of performing client care activities, including lifting, transferring, and operating mobility equipment.

Work Environment: Occasional evening and weekend hours may be required.
