360°kids is a vitally important community agency serving all of York Region. We have been a leader in the delivery of innovative support services for more than 35 years as we work to improve the lives of unhoused and marginalized youth and families in York Region. At 360°kids, we provide a unique continuum of care approach with programs and services that are essential to the health and well-being of youth. These proven strategies help to keep youth safe by providing stable housing options with 360° wraparound supports.
The Prevention Worker will act as a Family and Natural Support and provide de-escalation and stabilization to families that are in a state of crisis, which may put the youth at risk of homelessness. The worker will establish and maintain a respectful, collaborative working relationship with youth and families and the youth-identified natural supports, providing support, information, modeling, and teaching skills that support effective parenting and healthy family functioning. These skills include, but are not limited to, parent effectiveness, communication, boundaries, behavior management, discipline, child development, relationship building, and effective problem-solving relating to goals.
As a member of the 360°kids Prevention Team, the Prevention Worker will also be responsible for meeting with all youth who access the Richmond Hill Youth Hub and other 360°kids housing programs to attempt to divert them from an experience of homelessness and support connection with Family or Natural Supports. All members of the Prevention Team are trained in the Night stop and FNS intake process.
Key Responsibilities :
- Supports individual youth with establishing and strengthening connections to identified family and natural supports
- Supports youth and families through crisis and developing maintenance plans
- Links individual youth and families to appropriate services locally and regionally.
- Maintains comprehensive knowledge of community services and resources, and provides consultation to community support agencies.
- Provides case management support and follow-up for community-based housing plans and facilitates coordination of services.
- When necessary, acts as intermediary between youth and family, and supports by facilitating family mediation sessions.
- Provides support, crisis intervention and conflict resolution as required.
- Advocates on youth’s behalf when necessary and acts as a positive role model for youth.
- Liaises with participants, parents, community partners, agencies, donors, coworkers, and volunteers. Develops positive relations with the communities being served by the program and with the general public.
- Meets with youth who access the Richmond Hill Youth Hub (Connections, STAY, and Home Base), SHINE and HOPE to complete the Diversion Assessment.
- Supports youth, their families, natural supports, and professional support team to actualize the Diversion Plan
- With the Program Manager, supports, creates and implements approved strategies and plans that will result in the achievement of the program’s goals and objectives.
- Educates 360°kids staff on resources available to support youth in the community.
- Liaises with community partners to fortify existing partnerships and ensure warm transfer for youth between programs.
- Attends training workshops and relevant conferences that pertain to the role.
- Follows appropriate policies and procedures of 360°kids. Ensures that the program operates in line with 360°kids’ mission, vision and value and activities comply with applicable standards and laws.
