Clinical Projects Partner

Temporary Full Time - Approximately 12 months

Reporting to the Manager, Clinical Services, the Clinical Projects Partner will work together with the clinical team in planning, implementing and delivering on clinical projects in accordance with strategic and operational plans, organizational policies and procedures and relevant legislation, standards and regulatory bodies.

What You’ll Be Doing :

You will be a welcomed member of the Long Term Care team working in a fast-paced environment where you will make a meaningful difference.

As a Clinical Projects Partner you will provide support to the homes in Ontario with quality improvement and assurance.

1. Medication Assistance Program

  • Assist in the implementation of the Medication Assistance Program leading project calls, conducting site visits and pre-implementation analysis
  • Deliver onsite and virtual education ensuring toolkit resources are implemented
  • Support the communities with their pre and post go-live activities
  • Evaluate the effectiveness of the program and makes enhancements as necessary

2. Quality Improvement and Assurance

  • Develop and implement action plans on clinical care areas as identified in the community
  • Participate in quality improvement initiatives and research projects aimed at enhancing resident outcomes and optimizing clinical processes
  • Conduct quality assurance activities in the communities
  • Support the implementation of the risk management framework for clinical programs and contribute to the monitoring, reporting on risk and action planning

3. Education

  • Provide orientation and ongoing professional practice development to community nursing leadership team members
  • Collaborate with community leaders to implement best practices, standards and protocols to deliver on clinical care excellence
  • Support development and execution of program evaluations
  • Assist with the development of compliance plans, in consultation with the care community team members

Who You Are :

  • You are a Registered Nurse (RN) in good standing with the College of Nurses of Ontario
  • Three (3) years of related nursing leadership experience as a Director of Care in Long Term Care
  • Ability to build consensus and gain support for strategic goals and priorities in a complex organizational environment
  • Certificate of Gerontological Nursing (Canadian Nurses Assoc.) preferred
  • Effective verbal and written English communication
  • Able to drive and / or travel to long term care communities across Ontario
  • Strong computer skills (MS Office, excel, etc.)

What You Need to Know :

  • Travel is required across Ontario to support Long Term Care sites on the ground level ( Travel Reimbursement Applies )
  • You will require an automobile with current insurance - mileage is reimbursed
