Location : Variouslocations across BritishColumbia

Asa Home Health Nurse you will provide personalized nursing care topatients in their homes promoting recovery and enhancing theirquality of life.

Your role will involve assessing patients needsadministering treatments and supporting families in a comfortableand familiarenvironment.

KeyResponsibilities :

  • Conductcomprehensive assessments of patients health status includingphysical exams medical history reviews and home environmentevaluations.
  • Developand implement individualized care plans in collaboration withhealthcare teams tailoring interventions to meet each patientsuniqueneeds.
  • Administermedications wound care and other treatments as prescribed ensuringadherence to safety and infection controlprotocols.
  • Monitorpatients progress and responses to treatment making adjustments tocare plans as necessary and communicating findings to healthcareproviders.
  • Educatepatients and their families about managing medical conditionsmedications and necessary lifestyle changes providing resources andsupport.
  • Maintainaccurate and uptodate patient records documenting assessmentsinterventions and patientoutcomes.
  • Coordinatecare with other healthcare professionals including physicaltherapists social workers and physicians to ensure comprehensivesupport.
  • Stayinformed about best practices in home healthcare and changes inregulations to provide highqualitycare.

Requirements

  • Bachelors degree in Nursing (BSN) preferred; an Associate s degree inNursing (ADN) may be accepted with relevantexperience.
  • Activenursing license in insert relevant state orregion.
  • Previousexperience in home health nursing or a related field ispreferred.
  • Strongclinical assessment and critical thinking skills to manage diversepatient needs in a homesetting.
  • Excellentcommunication and interpersonal skills to build rapport withpatients andfamilies.
  • Abilityto work independently and manage time effectively in a home healthenvironment.
