Summary

Providence Health Care (PHC) is a health and wellness resource for all British Columbians. We’re committed to treating those we serve and each other with compassion and respect.

We have 8,000 staff, 1,500 medical staff / physicians, 200 researchers, 1,600 volunteers.

Hospital at Home

Hospital at Home (HaH) is a new service designed to improve patient outcomes and increase patient satisfaction. It will enable eligible patients to receive acute-level care in their homes, rather than in the hospital.

How it works

Patients who meet specific eligibility criteria are transferred home where they will continue to receive necessary acute care.

Patients are still considered inpatients even though their care is provided in their home, not in the hospital. They receive daily, in-person care from members of a dedicated, interdisciplinary team of hospital clinicians, supplemented by virtual visits and remote patient monitoring.

In-person nursing and physician care is available 24 hours a day, seven days a week.

Fleet vehicles will be provided to our Hospital at Home nurses.

Position Overview

Within the context of a client and family centred model of care, and in accordance with the Mission, Vision, Values and strategic directions of Providence Health Care, the Registered Nurse promotes a safe, respectful, and civil working environment for patients, clients, families, visitors and staff.

Reporting to the Patient Care Manager, the Registered Nurse Hospital at Home :

As a Hospital at Home nurse you will support in :

  • Daily assessments of patients progress and recovery
  • Assist with and administer medications
  • Perform Blood tests and other diagnostic tests
  • Oxygen therapy
  • Wound care
  • IV therapy

Common diagnoses of Hospital at Home patients include :

  • Congestive heart failure
  • Pneumonia
  • COPD
  • Dehydration
  • Sepsis
  • Cellulitis

Qualifications / Skills and Education

Education, Training and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives, currently practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives.

Valid BC driver’s license. Local area travel requires use of a vehicle.

Duties and Responsibilities

3.1 Prevents facility hospital admission by providing nursing care to patients who require acute level care in their homes, or who are transferred home early from an acute care facility to complete their admission at home, or who are seen in emergency departments, and complete their entire acute care admission at home.

3.2 Reviews patient status through methods such as reviewing the patient / client chart and other written information, patient observation, interviewing patient / client and family members, completion of questionnaires, collateral information and consultation with peers and other members of the health care team.

3.3 Assesses patient problems and needs, plans, implements, evaluates and prioritizes nursing care and nursing interventions and develops the plan of care for patients / clients

using clinical judgement and evidence-based practice.

3.4 Provides nursing care according to identified policies and procedures, technology and PHC standards. Teaches self / home care skills to patient and / or family members.

3.5 Monitors patient / client progress, evaluates outcomes, reassesses if goals are not met, and modifies plan of care based on evaluation.

3.6 Provides virtual care and Home Health Monitoring in accordance with plan of care.

3.7 Records observations and reports problems and / or changes in patient / client condition to designated staff.

3.8 Provides direct nursing interventions for management of crisis and life threatening situations.

3.9 Documents information including assessments, plan of care, patient / client status and

progress and discharge plans in a manual and / or computerized environment.

3.10 Collaborates with other members of the nursing team, multi-disciplinary team, community care partners, patient and family.

3.11 Accompanies patients in medical transport vehicles as required by the employer.

3.12 Identifies and responds to patient and family learning needs, provides patient educational and reference materials, incorporates individual and / or group teaching activities into the care plan and nursing interventions, and / or makes referrals to other identified resources.

Leads group patient / family sessions as required and coaches patient / family to develop and maintain health promoting behaviours within the context of the care issue.

3.13 Provides direction to other staff such as Care Attendants and Licensed Practical Nurses with regard to patient / client care and facilitates effective team work.

Responsibilities include but are not limited to collegial sharing of information, demonstrating and / or providing guidance on related tasks, providing incidental or ad hoc input into staff performance, ensuring staff are aware of assignments and reassigning clinical / health related tasks.

3.14 Participates in nursing orientation and shares clinical knowledge with other nursing staff, nursing students and other members of the interdisciplinary team.

Participates in and attends in-service presentations and other educational programs as required.

3.15 Participates in planning, implementation and evaluation of patient care policies, procedures, education materials, and best practices through methods such as taking part in program and quality improvement activities including nursing practice council, clinical pathway development, research and education and other committees.

3.16 Supports nursing research through methods such as identifying areas for possible clinical nursing research projects, adapting research to practice and participating in research activities to enhance nursing practice.

3.17 Advocates for the patients / clients, to protect and promote their rights to dignity, respect, privacy, confidentiality, autonomy and access to information.

3.18 Performs other related duties as assigned.

Vaccination Flag

As per the current Public Health Orders (Long Term Care / Seniors Assisted Living Provincial Health Order and the Health Sector Order), as of October 26, 2021, all employees working for Providence Health Care must be fully vaccinated against COVID-19.

Proof of vaccination status will be required.
