About the Hospital at Home Program :

The Hospital at Home program caters to patients who meet specific medical stability criteria, providing them with hospital-level care within their residences rather than within the traditional hospital setting.

Despite receiving care at home, these patients are considered inpatients. They receive daily in-person care from a dedicated, multidisciplinary team of hospital clinicians, supplemented by virtual visits and remote patient monitoring.

The Hospital at Home care is short-term and accessible around the clock, every day of the year.

A healthcare team, including a doctor, nurse, and other specialists, delivers care to the patient through a combination of in-person home visits and virtual visits (via phone and / or video).

As a Registered Nurse in the Hospital at Home team you will :

  • Collaborate within an interprofessional team and consult with physicians, other health care professionals, and the patient / family / caregiver, while providing nursing care to patients who require acute level care in their homes.
  • Assess, identify, plan, implement and evaluate the nursing care required to assist patients / clients with acute health care needs in the home and / or hospital setting using an established framework for nursing practice.
  • Develop, implement, evaluate and revise care plans, participate in discharge planning, function as a patient advocate, and teach patients and their caregivers about illness / injury and overall health promotion and wellness, and / or makes referrals to other identified resources.
  • Promote the best quality of life from the perspective of the patient / family.
  • Document care provided according to College and organizational standards.
  • Respond and address incidents in accordance with established policies and procedures.
  • Maintain and advance your own clinical competence, and participate on designated hospital committees / teams and approved research projects as assigned.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Three (3) years’ experience, including two (2) years’ recent, related acute medical / surgical experience or an equivalent combination of education, training and experience.
  • Valid BC Driver’s License required. Local area travel may require the use of a vehicle.

Knowledge, Skills & Abilities

Demonstrated ability in nursing practice related to designated client group - acute medical / surgical; including but not limited to patients with complex diseases, respiratory therapies, IV therapies, and nutritional therapies.

Demonstrated knowledge of pathophysiology, treatment protocols, and nursing interventions related to common diagnosis / surgeries and specific clinical situations related to area of practice (e.

g. Sepsis; Chronic Heart Failure; Acute Respiratory illness / disease; Cellulitis).

Demonstrated ability to provide nursing assessments & interventions related to common diagnosis and anticipate and respond to potential complications and care issues relevant to the designated patient group (e.

g. Pain (Acute / Chronic / Neuropathic); Cardiac / Respiratory arrest; Shock; Seizures).

  • Demonstrated ability to perform focused secondary assessments and integrate information from a variety of sources including diagnostics to develop plan of care relevant to the designated patient group.
  • Demonstrated ability in correct usage of equipment and maintains competency in working with vascular access devices including phlebotomy, peripheral IVs, peripherally inserted central venous catheters, and advanced vascular access devices such as tunneled central lines.
  • Demonstrated ability to document patient care on appropriate systems / forms in accordance with organizational and unit standards in a timely and accurate manner.
  • Demonstrated skill in safe medication practices, including obtaining best possible medication history (BPMH), order verification, and medication preparation and administration.

Accesses appropriate resources to support safe medication administration such as the program clinical pharmacist, parenteral drug therapy manual (PDTM), Lexicomp, and policies and decision support tools (DST).

Demonstrated skill in safe care & management of patients with respiratory therapies; IV medications and fluids; nutritional therapies;

wound care; drainage systems.

  • Demonstrated skill in CPR techniques.
  • Demonstrated ability to assess risk and limitations in relation to care and interventions in the home environment and to integrate and evaluate pertinent data (from multiple sources) to problem-solve effectively.

Ability to escalate as required.

  • Ability to operate a variety of technologies, including devices such as cell phone and laptop computers. Ability to teach and communicate with patients / family with the use of remote nurse call systems.
  • General of knowledge Trauma Informed Practice and Harm Reduction approaches and Strengths-based care.
  • Demonstrated ability to promote Indigenous Cultural Safety.
  • Demonstrated ability to provide competent and culturally safety care in a variety of settings and with diverse populations.
  • Physical ability to perform the duties of the position.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per the current Public Health Orders, as of October 5, 2023, all employees working for Vancouver Coastal Health must be fully vaccinated for COVID-19 or have received a single dose of the most-recent, updated COVID-19 vaccine.

Proof of vaccination status will be required.
