OVERVIEW : Reporting to the Memengwaa Supervisor, the Transitional Youth Worker provides case management supports to youth on an extension of care who are living in or transitioning out of Memengwaa transitional housing units. The key objective of this role is to support youth in building their personal capacity in preparation for the next stages of their lives.
KEY RESPONSIBILITIES :
- Provide case management services to Indigenous youth in and from care.
- Screen referrals and complete intakes to the program alongside team staff.
- Use program tools to complete SMART goal setting and life skill assessments with tenants; follow up on goals and assessments regularly.
- Conduct weekly check-ins with youth to review and update goals.
- Refer to and liaise with appropriate resources; advocate for youth when needed.
- Collaborate with youth support services to develop and provide comprehensive support based on identified needs.
- Provide written reports and work closely with SECFS case workers as required.
- Maintain youth files by completing case logs, keeping copies of critical information, and ensuring confidentiality.
- Attend meetings and appointments with youth when appropriate; empower youth to access resources independently.
- Conduct suite checks and encourage youth participation in household requirements, events, and activities.
- Provide ongoing support through check-ins and follow-ups when youth move out of the program.
- Intervene in crisis situations calmly and effectively.
- Collaborate with Memengwaa staff to develop programming calendars.
- Facilitate or coordinate programming, document attendance, and complete workshop reports.
- Recommend staff schedules in collaboration with the program manager.
- Participate in team meetings and training sessions as required.
