The Outreach Team Leader is responsible for overseeing the day-to-day management and operations of programs and services in consultation with the Coordinator of Outreach Services while ensuring a healthy and safe environment.
This position is one of direct support and requires the incumbent to provide all necessary assistance to the members including emotional and physical support along with crisis intervention during their schedule shift.
This is a full-time position at 40 hours per week. Work schedule varies.
KEY RESPONSIBILITIES :
Safety
- Ensure processes are being followed so that members and staff live and work in a safe environment.
- Ensure compliance with all safety rules and regulations are being adhered to by all, including but not limited to, working alone, food, medications, equipment, storage and maintenance.
- Provide crisis intervention for members as required. Conduct checks to ensure the safety of the facility and members throughout their shifts.
- Complete checks-ins with a designated contact in accordance with Community Venture policy and procedure.
- Understand and promote members’ rights and responsibilities.
- Provide transportation to and from various locations while demonstrating safe driving skills.
- Follow all traffic laws while driving defensively to ensure safety.
- Report concerns promptly to Coordinator of Outreach Services.
- Ensure proper lifting, twisting and reaching techniques are used during job performance.
- Use PPE provided when necessary.
Health and Medical
- Ensure individual’s health and medical needs are accurately attended to in a timely fashion, and that all medication is properly administered and recorded (where applicable).
- Recognize and report concerns of abuse or neglect.
- Provide emotional support for members as required.
- Assist Coordinator in developing meals and meal plans, ensuring nutritious meals and snacks are prepared and provided in accordance with Canada’s Food Guide and adapted to individual likes, preferences, choices and medical or religious requirements (where applicable).
Person Centred Plans
- Be involved in implementing and monitoring individual plans.
- Provide individuals with support in pre-planning by providing a mechanism to gather information on an individual's goals and desired outcomes.
- Communicate necessary information and maintain positive relationships with other agencies or programs.
Individual Personal Development
- Encourage and assist each individual to participate in daily activities in their homes, in employment and in the community including reinforcing work and community expectations, personal hygiene, meal preparation, grocery shopping, laundry and the care of the house and yard.
- Ensure that the environment of the program is supportive, nurturing and encourages a high degree of member involvement including decision making.
- Role model appropriate behaviour and attitude supporting growth and promoting independence for the members.
Public Relations and Communication
- Responsible for the wise use of member and program resources.
- Develop opportunities for relationships with families and the community and assist individuals’ to develop positive social interactions.
- Ensure all contact with others is courteous and professional.
- Treat all members and staff with respect and dignity at all times.
Program Development
- Assist the Coordinator in creating, managing and facilitating appropriate activities, special events and spiritual care programs according to member interest, skill and ability levels.
- Promote social interaction and personal development.
- Ensure all necessary components and any required documentation is completed for assigned activities.
Other Duties as Required
QUALIFICATIONS AND EDUCATION REQUIREMENTS :
Education, Qualifications and Certifications :
- The successful job applicant will have completed Community College diploma / University degree or equivalent experience in a community based facility, preferably in some field of disabilities.
- Valid Manitoba Class V driver license and access to a personal vehicle
- Criminal record check
- Child Abuse Registry check
- Adult Abuse Registry check
- Acceptable Driver’s Abstract
Experience and Skilled Knowledge Requirements
Suitable candidates should have at least two years’ experience in developmental services. Experience in the field of disabilities (will be considered an asset).
PREFERRED SKILLS / CAPABILITIES :
- Moderate amount of keyboarding / manual record keeping
- Occasional to moderate amounts of physical care (including manual or assisted transfers)
- Moderate lifting / packing of member belongings, program and / or residence supplies
- Frequent standing, sitting, walking, & reaching
- Moderate amount light housekeeping and property maintenance
- Occasional use of physical behaviour intervention techniques.
- Attentive listening & observation of body language
- Continuously monitoring the environment
- Significant reading, writing, arithmetic, and computer skills
