In this dynamic role, you will assess client situations, implement effective care strategies, and collaborate with healthcare providers to facilitate positive outcomes. Your expertise in mental health treatments and cultural safety will significantly impact the recovery process for clients and their families.
Key Responsibilities:
• Assess and provide short-term counseling to patients and caregivers
• Develop partnerships with interdisciplinary health teams
• Coordinate continuity of care and client transitions
• Conduct thorough risk assessments and mental health evaluations
• Document care plans and progress using digital systems
Requirements:
• MSW from an accredited institution and recent clinical experience
• Full registration with the British Columbia College of Social Workers
• Access to a vehicle with a valid BC Driver’s License
• Strong understanding of health care ethics and relevant legislation
• Ability to work independently and collaboratively
