As an integral member of the Hospital leadership and patient care teams, the Senior Clinical Program Director (SCPD) works in partnership with the program medical directors in providing exemplary leadership and strategic guidance to the Rehabilitation and Palliative Program. The primary responsibility of the SCPD is to ensure excellence in the provision of patient care in day-to-day operations as well as longer term program planning for improvements, the achievement of corporate / program objectives and the creation of a healthy workplace that maximizes the potential of all who work at Unity Health Toronto.
On August 1, 2017, Providence Healthcare, St. Joseph’s Health Centre and St. Michael’s Hospital integrated to form a new health network known as Unity Health Toronto. The health network provides services spanning primary care, secondary community care, tertiary and quaternary care services to post-acute through rehabilitation, palliative care and long-term care. The Providence Healthcare site is embarking on a journey to become a centre of excellence in rehabilitation science and healthy living to provide the best possible care for the patients we serve.
The SCPD of the Rehabilitation and Palliative Program leads the Providence Healthcare site specific Rehabilitation Program with the opportunity to create and support future services at the St. Michael’s Hospital and St. Joseph’s Health Centre sites. This role will also have cross-site program with accountabilities spanning palliative clinical services across all three sites. The Providence site provides rehabilitation inpatient services as well as rehabilitative outpatient services to individuals with musculoskeletal, stroke, and neurologic medical needs. All three sites provide pain, symptom management, and end of life care to patients with palliative care needs – in addition, Providence provides caregiver relief and support programs for these populations. We provide services to many living in poverty and facing many hurdles in achieving recovery and community re-integration and we are committed to supporting recovery through timely access to high quality, evidence informed comprehensive health and social supports. Robust community based and community partnered programs and services are key the care we deliver.
The SCPD leads the strategic planning of all services in the Rehabilitation and Palliative Programs and maintains responsibility for ensuring the efficient / effective delivery of the clinical program including all aspects of planning, human resource and financial management. This involves facilitating program development and strengthening program vision, and promoting a culture of discovery and innovation in all clinical and non-clinical activities within the Programs and the development of external relationships with the community partners and stakeholders in patient care delivery.
