The Role:We are hiring a Community Health Representative (CHR) to lead growth and community engagement in the Tri Cities area. (Coquitlam, Port Coquitlam, Port Moody, Maple Ridge & Pitt Meadows).
This position is ideal for someone who:
  • Loves being on the road and building real relationships.
  • Is confident conducting in-home assessments.
  • Thrives in independent, performance-based environments.
  • Understands that trust is built face-to-face.
You will spend approximately:
  • 90% of your time in the field visiting referral partners and meeting families.
  • 10% of your time in your home office updating CRM and managing follow-ups.

What You’ll Be Responsible For:
Community Relationship Development:
  • Build and maintain referral partnerships with occupational therapists, hospital discharge planners, social workers, physicians, pharmacists, assisted living sites, and other healthcare professionals.
  • Represent Just Like Family at community events, networking meetings, and healthcare touchpoints.
  • Increase brand awareness and position our company as the trusted local home care provider.

Client Assessments & Sales Leadership:
  • Conduct in-home care assessments with families.
  • Create personalized care plans aligned with client needs.
  • Guide families confidently through the decision-making process.
  • Manage your sales pipeline from first contact to service start.

Accountability & Performance:
  • Track all leads, referrals, and follow-ups in Monday.com (or similar CRM).
  • Stay accountable to monthly performance targets.
  • Provide consistent reporting and communication with leadership.

Who You Are:
  • Experienced in sales, community outreach, or face-to-face marketing.
  • Healthcare or home care experience is an asset.
  • Highly organized and self-managed.
  • Comfortable working independently without daily supervision.
  • Confident speaking with professionals and families in sensitive situations.
  • Resilient and motivated by results.
  • Strong written and verbal communication skills.
  • Tech-comfortable (CRM systems, tracking, documentation).

Requirements:
  • Valid driver’s license.
  • Reliable vehicle (mileage reimbursed).
  • Comfortable with frequent local travel.
