Nous sommes à la recherche d’un(e) gestionnaire de cas infirmier(ère) bilingue sur le terrain (IA) pour soutenir les patients, les professionnels de la santé et les partenaires cliniques dans le cadre d’un programme de soutien aux patients (PSP) axé sur les thérapies spécialisées et biologiques.

Dans ce rôle terrain, vous agirez à titre de ressource clinique clé et de principal point de contact tout au long du parcours du patient. Vous aiderez les patients à accéder à leur traitement, à mieux comprendre leur thérapie et à demeurer engagés dans leur plan de soins. Vous collaborerez étroitement avec les médecins, les équipes cliniques, les patients, les pharmacies et les intervenants internes afin d’offrir une expérience patient exceptionnelle tout en soutenant des cliniques partout dans la région du Grand Toronto (RGT).

Ce poste exige des déplacements réguliers vers des cliniques et établissements de soins de santé situés dans la RGT.

Ce que vous ferez

  • Agir à titre de principal contact clinique pour les patients, les médecins et le personnel clinique inscrits au programme de soutien aux patients.
  • Soutenir l’intégration des patients au programme et faciliter l’accès rapide aux médicaments spécialisés prescrits.
  • Offrir de l’enseignement aux patients sur les maladies, les attentes liées aux traitements, l’administration des médicaments, la gestion des effets secondaires et les stratégies favorisant l’adhésion au traitement.
  • Fournir de la formation sur les injections et l’autoadministration en clinique, en mode virtuel ou à domicile, selon les besoins.
  • Collaborer avec les professionnels de la santé afin de coordonner les soins et de communiquer les mises à jour pertinentes.
  • Soutenir les activités liées au remboursement et à l’accès aux traitements, notamment en recueillant la documentation requise et en appuyant les processus d’inscription des patients.
  • Réviser les dossiers des patients et recueillir les renseignements nécessaires aux activités de remboursement ou aux programmes cliniques.
  • Assurer le suivi de la progression des patients et favoriser l’adhésion au traitement tout au long de leur parcours thérapeutique.
  • Identifier les situations nécessitant un soutien clinique ou opérationnel additionnel et les transmettre aux parties appropriées.
  • Établir et maintenir des relations de confiance avec les médecins, le personnel clinique, les pharmacies et les patients.
  • Documenter avec exactitude les interactions avec les patients et les activités du programme conformément aux exigences établies.
  • Contribuer aux initiatives du programme et aux activités d’amélioration continue.

Exigences de base

  • Détenir un permis actif et sans restriction d’infirmier(ère) autorisé(e) (IA) en Ontario.
  • Maîtriser le français et l’anglais, tant à l’oral qu’à l’écrit.
  • Posséder une expérience préalable au sein d’un programme de soutien aux patients (PSP).
  • Détenir un permis de conduire valide de l’Ontario et avoir accès à un véhicule fiable.
  • Être en mesure et disposé(e) à se déplacer jusqu’à 60 % du temps (environ trois jours par semaine) dans la région du Grand Toronto.
  • Démontrer de solides compétences en éducation des patients et en communication.
  • Maîtriser la suite Microsoft Office et les systèmes de documentation électronique.

Compétences et expérience souhaitées

  • Détenir des permis d’exercice infirmier actifs dans d’autres provinces canadiennes.
  • Posséder de l’expérience dans les domaines de la pharmacie spécialisée, des médicaments biologiques, de l’immunologie, de la rhumatologie, de la dermatologie, de la gastroentérologie ou d’autres domaines thérapeutiques spécialisés.
  • Avoir de l’expérience en formation sur les injections et en éducation des patients.
  • Connaître les processus de remboursement, d’accès aux traitements ou les programmes liés aux médicaments spécialisés.
  • Avoir travaillé directement avec des cabinets médicaux, des cliniques de perfusion ou d’autres cliniques spécialisées.
  • Connaître les thérapies biologiques et les exigences préalables aux traitements, notamment les analyses de laboratoire, l’imagerie diagnostique et les protocoles de dépistage.
  • Posséder d’excellentes aptitudes pour établir des relations et gérer les parties prenantes.
  • Avoir de l’expérience dans un rôle autonome au sein d’un environnement de soins de santé sur le terrain.

Déplacements, environnement de travail et exigences physiques

  • Poste sur le terrain soutenant des cliniques dans l’ensemble de la région du Grand Toronto (RGT).
  • Déplacements requis environ 60 % du temps (3 jours par semaine).
  • Les frais de kilométrage et les dépenses de déplacement approuvées sont remboursés conformément aux politiques de l’entreprise.
  • Le reste du temps peut être consacré au télétravail à partir d’un bureau à domicile sécuritaire.
  • Doit être en mesure de se déplacer entre les cliniques, les cabinets médicaux et les lieux de rencontre avec les patients, au besoin.

About the Role

We're seeking a Bilingual Field Nurse Case Manager (RN) to support patients, healthcare professionals, and clinic partners through a Patient Support Program (PSP) focused on specialty and biologic therapies.

In this field-based role, you'll serve as a key clinical resource and primary point of contact throughout the patient journey, helping patients access therapy, understand their treatment, and remain engaged in care. You'll collaborate closely with physicians, clinic teams, patients, pharmacies, and internal stakeholders to deliver an exceptional patient experience while supporting clinics across the Greater Toronto Area (GTA).

This role requires regular travel to clinics and healthcare sites throughout the GTA.

What You'll Do

  • Serve as the primary clinical contact for patients, physicians, and clinic staff enrolled in the Patient Support Program.
  • Support patient onboarding and facilitate timely access to prescribed specialty medications.
  • Deliver patient education on disease states, treatment expectations, medication administration, side effect management, and adherence strategies.
  • Conduct injection and self-administration training in clinic, virtually, or in-home as required.
  • Collaborate with healthcare providers to coordinate patient care and communicate relevant updates.
  • Assist with reimbursement and patient access activities, including gathering required documentation and supporting patient enrollment processes.
  • Conduct patient chart reviews and collect information required for reimbursement or clinical program activities.
  • Monitor patient progress and support adherence throughout the treatment journey.
  • Identify and escalate issues requiring additional clinical or operational support.
  • Build and maintain trusted relationships with physicians, clinic staff, pharmacies, and patients.
  • Document patient interactions and program activities accurately and in accordance with program requirements.
  • Support ongoing program initiatives and continuous improvement activities.

Basic Requirements

  • Active and unrestricted Registered Nurse (RN) license in Ontario.
  • Fluency in both French and English (written and verbal).
  • Previous experience working within a Patient Support Program (PSP).
  • Valid Ontario driver's license and access to a reliable vehicle.
  • Ability and willingness to travel up to 60% of the time (approximately 3 days per week) throughout the GTA.
  • Strong patient education and communication skills.
  • Proficiency with Microsoft Office and digital documentation systems.

Preferred Skills & Experience

  • Additional active nursing licenses in other Canadian provinces.
  • Experience supporting specialty pharmacy, biologic, immunology, rheumatology, dermatology, gastroenterology, or other specialty therapeutic areas.
  • Experience providing injection training and patient education.
  • Knowledge of reimbursement, patient access, or specialty medication programs.
  • Experience working directly with physicians' offices, infusion clinics, or specialty clinics.
  • Familiarity with biologic therapies and pre-treatment requirements, including laboratory testing, diagnostic imaging, and screening protocols.
  • Strong relationship-building and stakeholder management skills.
  • Experience working independently in a field-based healthcare role.

Travel, Work Environment & Physical Requirements

  • Field-based position supporting clinics across the Greater Toronto Area (GTA).
  • Travel required approximately 60% of the time (3 days per week).
  • Mileage and approved travel expenses are reimbursed in accordance with company policy.
  • Remaining time may be spent working remotely from a secure home office environment.
  • Must be able to travel between clinics, physician offices, and patient locations as required.

$56,800 - $75,700
