As a member of our inter-professional team, the Nurse Practitioner will utilize clinical expertise and in-depth knowledge of children, adolescents, their families, and adults to plan and evaluate patient care.
The successful candidate will provide leadership and promote excellence in nursing by offering expertise in clinical practice, consultation, education, and research in addiction. The Nurse Practitioner will integrate research evidence into practice to ensure best practice standards are maintained and incorporated into clinical initiatives.
The principal responsibilities include identifying, screening, assessing, consulting, and providing clinical intervention to individuals with Mental Health & Substance Use Disorders and their families. This involves providing specialized assessment, coordination, and treatment for individuals living with these disorders. The clinician offers counseling, education, and support services to individuals, families, and the community, helping clients understand and address related problems. Collaboration with an inter-professional team within the Mental Health Program and establishing partnerships with community organizations are essential. This role is part of the addiction services within Mental Health.
Qualifications
Halton Healthcare Services seeks leaders in their profession. The successful candidate must be a team player with effective communication skills, patient-focused, caring, respectful, and fiscally responsible. Requirements include :
- Master’s Degree in Nursing / Master of Science in Nursing or recognized equivalent (preferred)
- Current registration in the Extended Class with the College of Nurses of Ontario, preferably with Primary Health Care specialty
- Nurse Practitioner’s Certificate from a recognized program
- Membership in professional associations such as CNA, RNAO, or NPAO (preferred)
- Completion or willingness to obtain the Opioid Dependence Treatment Certificate within 1 year
- Minimum three years’ experience in substance use disorders / addiction
- Minimum five years’ experience in Mental Health, including inpatient and community services
- Thorough knowledge of substance use, disorders, evidence-based treatments, and application to diverse populations
- Expertise in bio-psychosocial assessments and formulation skills for person-centered, strength-based strategies
- Experience with motivational interviewing and trauma-informed care
- Provision of opioid addiction treatment with harm reduction approach
- Ability to link clients to primary care and foster relationships to meet client needs
- Knowledge of counseling methods for individuals, families, groups, and crisis situations
- Skill in working with families affected by mental health & substance use disorders
- Experience with concurrent disorders and harm-reduction frameworks
- Experience working with inter-professional teams in fast-paced hospital settings
- Understanding of cultural safety and diversity
- Ability to tailor interventions to clients’ and families’ circumstances
- Partnership experience with primary care and community providers
- Ability to work independently
- Proficiency with MS Office and electronic health platforms
- Satisfactory employment and attendance record
- Experience in psychoeducation and psychotherapy (preferred)
Hours : Monday to Friday, 0800-1600 (subject to change)
