The Specialist – Palliative Care Implementation is responsible for project management support for the implementation of the Palliative Care Health Services Delivery Framework (“The Delivery Framework”), initially focused on model of care recommendations for adults in community-based settings in their OH Region. They are accountable for coordinating and tracking regional work to ensure timely completion of deliverables. They will support implementing organizations (e.g., LTCH, OHT, FHTs, HSPs) to advance palliative care through quality improvement initiatives, to monitor risks and to sustain the spread and scale of successful initiatives. Where Indigenous communities wish to be involved, they will provide support to Indigenous communities and Indigenous organizations for their implementation efforts.
Please be advised that the scope of this role is regional in nature and therefore, will focus on the particular needs of the Northwest Region (Thunder Bay).
