Position Summary

The Seniors Mental Health Consultant provides psychogeriatric services and education on mental health issues to clients and families related to older adults.

Duties

• Provide comprehensive assessment consultation and education to geriatric / psychogeriatric clients and their caregivers. Assessments are completed in-home community and / or a clinical setting.
• Participate in case conferences with Clinical Consultant (Psychiatrist)
• Prepare written reports outlining intervention strategies
• Establish liaisons with appropriate community agencies organizations and committees whose objectives correspond to those of the service.
• Ensure timely and accurate clinical documentation and completion of workload
• Make appropriate referrals and provide guidance to clients and families as required
• Participate in program planning and evaluation
• Participate in the development and provision of educational materials and programs for clients families and community service providers
