Wound Care Specialist

What will you do?

The Wound Care Specialist is responsible for providing support, treatment, product recommendations and follow‑up care for patients with identified problem wounds or ostomies. As a key resource, the specialist will interpret and apply evidence‑based best practice to improve the quality of patient care; and conduct outreach to clinicians and health care providers in order to facilitate adoption of services, problem solving and understanding of clinical guidelines as they pertain to identified wound care and ostomy patients.

Primary Responsibilities

  • Perform Champlain‑wide clinical wound and ostomy management in collaboration with members of the patient’s care team, including direct client consultation and evaluation of individual care plans.
  • Act as a resource for Care Coordinators and service providers to assist in meeting patients’ needs regarding complex wound/ostomy situations with heightened risk for negative outcomes.
  • Support the development of internal policies and procedures for wound and ostomy management.
  • Participate in establishing, maintaining, and monitoring standards for the wound and ostomy program, including external and internal committee work and quality initiatives.
  • Access research on wound and ostomy products and their application to make sound recommendations for treatment/care.
  • Handle special requests for wound care supplies, request pricing and coding in the wound care supplies catalogue, and approve off‑formulary requests.
  • Provide recommendations on alternative products within the wound care supplies catalogue.
  • Participate in wound and ostomy program data collection, analysis, evaluation and recommendations.
  • Monitor negative pressure wound therapy patients to ensure compliance with OH aHome Champlain guidelines.
  • Answer questions or concerns regarding wound or ostomy care from patients, caregivers, and service providers.
  • Other related duties may be assigned.

Hours of Work

Monday to Friday – 8:30 am to 4:30 pm (70 hrs/bi-week)

Compensation

Starting Salary: $44,480 – $51,340

Education and Experience

  • Registered Nurse (RN) (BScN or diploma) with a current Certificate of Registration from the Ontario College of Nurses.
  • Completion of “Nurse Specialized in Wound, Ostomy, & Continence” (NSWOC) (formerly ETN).
  • At least five (5) years of related professional nursing experience, including wound care and ostomy management.
  • Membership with “Nurses Specialized in Wound, Ostomy, & Continence Canada” (NSWOCC) (formerly CAET) considered an asset.
  • Adult teaching experience and/or adult education courses considered an asset.

Knowledge

  • Wound care and ostomy management.
  • In‑depth knowledge of wound and ostomy supplies to make recommendations for the supplies catalogue and address substitution requests.
  • Comfort with emerging technology and innovative technology (e.g., tablets, e‑referral, e‑health coaching).
  • Knowledge of the healthcare system and the role of OH aHome Champlain within it.
  • Knowledge of OH aHome Champlain operations, quality, risk, and principles.
  • Knowledge of relevant healthcare legislation.
  • Knowledge of OH aHome Champlain policies, procedures, strategic direction and goals.
  • Understanding of OH aHome Champlain partners and contracted service providers in the community.

Skills and Abilities

  • Effective planning and organizing skills to manage multiple clients and take corrective action as required.
  • Effective interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability.
  • Ability to manage difficult conversations with physicians, nurses and other interdisciplinary team members in a healthy and effective manner.
  • Strong ability to work effectively both in a team environment and independently.
  • Clear and concise communication, both oral and written, including presentation skills.
  • Excellent listening and facilitation skills necessary for working with patients and caregivers.
  • Awareness of cultural diversity.
  • Time‑management and prioritization skills, with flexibility in a dynamic environment.
  • Diplomacy in sensitive and/or confidential issues.
  • Accurate completion of required documentation, reports, forms, and recommendations.
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Outlook, Word, Excel, PowerPoint).
  • A valid driver’s license and access to a vehicle.
