Duties
may include preparation of paperwork, case conferences, liaising with treatment providers and community case managers, and providing support to family members and patients who are supported by Community Treatment Orders
- Evaluate the effectiveness of the individualized CTO plan.
- Develop and maintain necessary working relationships with hospitals and community service providers and programs who will assist with Community Treatments Orders and form the basis of the discharge plan into the community.
- Develop and maintain a database on client demographics, referrals and outcome data.
- Monitor and evaluate information regarding the usage of Community Treatment Orders, gaps in the system related to development of CTOs and the identification of program and service needs as identified by the data and stakeholders. Report these findings to appropriate authorities.
- Participate with other regional CTO Coordinators in regular meetings.
- Develop protocols in conjunction with the other regional CTO Coordinators to be used to set up Community Treatment Orders including the involvement of key stakeholders (hospitals, police, mental health providers) including a feedback mechanism for and to those agencies.
- Develop a close liaison with community Mental Health Case Managers who provide services to the CTO clients to ensure regular communication amongst the treatment team.
- Provide to the hospitals/community ongoing training and education with respect to the nature of Community Treatment Orders, their use in community, routes for accessing same etc. Provide advocacy on behalf of clients and families experiencing difficulties with Community Treatment Orders.
- Document on patient’s clinical record according to hospital, college, and legislative standards.
- Maintain workload measurement and data collection.
- Participate and lead in research and program evaluation.
- Participate in teaching and education of Social Work students and support students from health-related disciplines.
- Participation in hospital Social Work and quality improvement requirements.
- Attend Social Work meetings.
- Participate in Clinical audit and peer consultation groups
- Keep current of hospital policies and procedures and applicable legislation.
- Keep up to date regarding professional standards by reviewing current literature, attending educational events, networking with colleagues, etc.
- Provide educational presentations to community groups.
- Required to travel within the Champlain Health District.
QUALIFICATIONS:- Master’s degree in social work from an accredited university
- Current registration and member in good standing with the Ontario College of Social Workers & Social Service Workers.
- Knowledge of the Mental Health system, the Mental Health Act, the role of the Public Guardian and Trustee’s Office, the role of the Rights Advisor and other related Mental Health Practitioners. Knowledge of the Psychiatric Advocate Office, Patient representatives, Health Care Consent Act, Substitute Decision Act.
- Minimum 3 to 5 years clinical psychiatric experience in the community and in a Mental Health Care or Chronic Care setting.
- Access to reliable transportation and valid driver’s license is required.
- To work in a manner that is in compliance with staff and patient/client safety practices, policies and procedures of the ROHCG.
- To ensure a work environment that is conducive to the ROHCG Respectful Workplace policy.
- English level A- in oral expression, oral comprehension, reading and writing is mandatory. Bilingual (French/English) level A- is considered an asset
Salary Range: 44.67 to 57.90 per hour
