  • Practice in accordance with the Regulated Health Professions Act, Gerontological Nurses Association (GNA) Standards, The Long-Term Care Homes Act, the Practice Standards set by the College of Nurses of Ontario and standards
  • Perform detailed holistic assessment of clients with wound, ostomy, and / or continence-related concerns
  • Formulate treatment recommendations in collaboration with the interprofessional team, and develop a plan of care
  • Lead advances in clinical practice through the provision of clinical expertise and education related to skin, wound care, for acute and chronic wounds, and stoma management
  • Demonstrated teaching skills and expertise in preparing effective teaching tools and methods are preferred
  • Provide leadership and promote excellence in patient-centred nursing care through expert practice, education, consultation and research
  • Actively participate in a Regional NLOT Community of Practice
  • Provide input and support in the development and implementation of integrated hospital-community-LTCH clinical pathways
  • Develop linkages with external partners as appropriate to ensure continuity of care
  • Provide direct patient care to LTC residents
  • Utilize the Clinical Information System
  • Utilize the protective equipment provided and identifies and report hazards
  • This position will work in off-site community settings providing clinical care to support residents living in Long-Term Care (LTC) homes within a specific sub-regional boundary in Scarborough

Responsibilities :

  • Apply clinical knowledge and leadership skills to collect and interpret information, make appropriate clinical decisions, and conduct therapeutic nursing interventions.
  • Provide care that is characterized by up-to-date, ethical, and resource-efficient clinical practice and effective communication in partnership with patients, families, and clinical teams.
  • Must have demonstrated leadership skills, excellent critical thinking and the ability to adapt to unpredictable situations.

Requirements :

  • Bachelor s Degree in Nursing
  • Current Registered Nurse registration with the College of Nurses of Ontario
  • Current Basic Cardiac Life Support (BCLS) certification required
  • Completion- Nurses Specialized in Wound, Ostomy and Continence Canada (NSWOCC) WOC-EP Program
  • Acute medicine experience within the last two years
  • Wound and Ostomy Care experience within the last two years
  • Advanced clinical knowledge related to Pressure Injury Prevention
  • Excellent interpersonal and communication skills, both written and verbal
  • Demonstrates excellent problem-solving, decision making and critical-thinking skills
  • Demonstrates excellent organizational, time-management and team-building skills
  • Demonstrates initiative and the ability to function both independently and as an inter-professional team member
  • Flexible and adaptable regarding assignment locations
  • Demonstrates a commitment to customer service when dealing with patients,

colleagues and volunteers and other members of the community and public

Demonstrates values consistent with SHN s Code of Conduct and is committed to

fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.

Demonstrate an understanding of and commitment to SHN's comprehensive safety

programs and practices, including staff safety, patient safety, and environmental

safety and participates in regular in-service education in this area.

Valid Ontario Driver's license and access to a car required
