Established in 1978, SWCHC provides a wide range of health care services such as primary health care, health promotion, community development, mental health, and harm reduction. What the Program Entails

Our Primary Health Care team delivers high-quality, comprehensive services both at the Centre and through community outreach. As a Community Health Centre, we are mandated to serve populations that experience barriers to accessing health care, including people who are homeless, seniors, refugees, newcomers, and those living on low income.

CHCs integrate clinical services with health promotion and community programs under one roof, addressing the full range of factors that influence health and well-being.

What Youll Do

Reporting to the Director of Integrated Health Services, the Nurse Practitioner (NP) is a key member of the interdisciplinary team, delivering comprehensive primary health care services within the Primary Health Care (PHC) clinic. The NP practices to their full scope, providing care through same-day access and/or a rostered client model to meet program needs. They deliver culturally responsive care both independently and in collaboration with an interprofessional team. The successful candidate will demonstrate the ability to work autonomously and collaboratively, with a strong commitment to culturally safe, equitable, and client-centred care. This position requires availability to provide service one evening per week, until 8:00 p.m.

The NP is responsible for delivering primary health care services in accordance with the Standards of Practice for Registered Nurses in the Extended Class and the Expanded Nursing Services for Patients Act. This includes health promotion, counselling, disease prevention, diagnosis, treatment, and referrals to specialists or diagnostic testing. Providing clinical services requires the NP to manage a diverse range of health conditions while addressing the complex needs of the populations served.

The NP plays an active role in the development, implementation, monitoring, and evaluation of programs and services, ensuring accessible and high-quality care for individuals with diverse health, social, cultural, and economic needs. Functioning both autonomously and collaboratively, the NP contributes to advancing program goals and enhancing health outcomes within the community.

What Were Looking For

Your Education Path

  • Current extended class certificate (NP-PHC) and unrestricted registration with the College of Nurses of Ontario
  • Knowledge of the ODSP application process
  • Experience with using Practice Solutions EMR is an asset
  • Certification in CPR (BLS-C)

Life and Work Experience

  • Minimum two years experience as a Nurse Practitioner working in a primary care setting
  • Strong clinical skills and diagnostic abilities for clients across the life span
  • Previous experience working with culturally diverse and marginalized populations
  • Interest in providing outreach services

Required Language

  • Fluency in English required.
  • Working knowledge of other languages is an asset.

Your Day-to-Day at a Glance

  • The Nurse Practitioner (NP) provides a comprehensive range of services, including triage, physical assessment, diagnosis, treatment, counselling, health promotion, and preventative care, tailored to clients with diverse health, social, cultural, and economic needs. Working to the full scope of practice within the Walk-in Clinic and/or a Rostered practice, the NP will be expected to function both independently and as part of an interprofessional team, delivering care in a culturally sensitive manner. The successful candidate may also be required to provide services during one evening per week until 8:00 p.m.
