Social Worker, MSW Healthy Families (Full Time Permanent)
The Role and Opportunity
The Canadian Mental Health Association - Toronto is looking for a Regulated Health Professional (Social Worker) for the Healthy Families Program .
The Healthy Families Program supports caregivers who have children who are being supported by the Children’s Aid Society of Toronto (CAS-T) and are struggling with mental health and / or addiction issues.
This team is a multidisciplinary team providing community-based assessment, short-term support and linkages. Referrals come from CAS-T branch and the program is based at the CAS-T office located on Isabella Street.
Salary : $62,915 $78,748

Hours : 35 per week

Responsibilities
Reporting to the Program Manager, the Social Worker will work with the interdisciplinary team to support their clients. They will be required to participate in team meetings and provide regular updates to managers as well as providing education and consultation to CAS works as required.
The aim of the Social Worker will be to assess the mental health needs, physical health needs, functional abilities and care options for the clients.
The role will also involve assessing the impact of mental health symptoms and substance use. They will be required to provide psycho-education and evidence-based interventions to the individual and their family including providing linkages to treatment for physical and mental health needs.
The Social Worker will work from a recovery based, trauma informed perspective to build and maintain a trusting, respectful and collaborative working relationship with client, including client-centered engagement, service provision, interventions, skills development and goal planning.
Experience & Education Required
CMHA Toronto’s ideal candidate will have a Master’s Degree in Social Work or equivalent and registration with a professional college.
- The candidate will bring a minimum of three (3) years of experience working with people with mental health and addiction issues in a community-based setting;
- knowledge and experience of evidence-based behavioral interventions (CBT / DBT), addictions treatment and approaches to trauma-informed care;
and experience conducting biopsychosocial assessments with a focus on family strengths and crisis intervention skills.
The candidate will bring excellent organizational, interpersonal and communication skills. They have the ability to work with diverse communities and demonstrate an anti-oppressive framework.
The candidate will also have the ability to work independently as well as within a dynamic team environment. The candidate will have proficiency in Microsoft applications including Office 365.
The candidate will be required to have a valid Ontario Driver’s License, access to a vehicle and insurance coverage.
