Georgian Nurse Practitioner Led Clinic
The Association of Family Health Teams of OntarioInnisfil, ON, CA- Full-time
Overview
The Georgian Nurse Practitioner‑Led Clinic (GNPLC) is a multi‑site primary care clinic in Barrie and Innisfil. Our team is comprised of nurse practitioners, registered nurses, registered dietitians, social workers and physiotherapists and supported by an administrative team. Together we provide access to team‑based primary care to over 6000 residents in our communities. Come join us in this new opportunity.
Nurse Practitioner (RN‑EC)
NPs function as the Most‑Responsible Provider (MRP) for a defined roster of patients. The NP will collaborate internally with clinical colleagues, and externally with specialist physicians and community services and agencies in the provision of primary health care for their patients. You will practice autonomously, providing periodic health examinations, pharmacological and counseling interventions, health promotion, and chronic disease management.
Ours is an environment where everyone works interdependently towards common goals. While we operate in a MRP model, support is always provided, both formally and informally, so you are never truly alone in providing care and where you are able to leverage the expertise of your colleagues.

- Current registration and in good standing with the College of Nurses of Ontario as a NP
- Two years of NP experience in a team based primary health care setting preferred
