Sommaire du poste

Le titulaire du poste est responsable de prodiguer des soins infirmiers professionnels à des personnes, y compris l’administration intraveineuse ou par injection de produits pharmaceutiques spécialisés. L’infirmier(ère) contribue à la promotion de la santé des patients en complétant des évaluations de santé avant les traitements, en révisant les dossiers des patients et en collaborant avec les médecins et les membres de l’équipe multidisciplinaire, au besoin.
L’objectif ultime est de fournir des thérapies médicales ou de l’enseignement sur l’auto‑administration aux patients, en clinique, à domicile ou dans un environnement virtuel.

Responsabilités spécifiques

  • Examiner les dossiers des patients afin de comprendre leur historique médical.
  • Évaluer l’état de santé des patients à l’aide d’un processus de présélection couvrant les contre‑indications ainsi que les données objectives et subjectives, afin de déterminer l’admissibilité aux thérapies médicales.
  • Manipuler et reconstituer des produits pharmaceutiques spécialisés conformément à la monographie du produit du fabricant et aux lignes directrices de Santé Canada.
  • Respecter les exigences de la chaîne du froid pour les médicaments, conformément aux politiques et procédures d’INVIVA et aux recommandations des fabricants.
  • Au besoin, effectuer l’insertion de cathéters intraveineux périphériques ou établir un accès par voie centrale selon les normes de meilleures pratiques.
  • Suivre les directives médicales propres à chaque patient pour l’administration des traitements et la prise en charge des effets indésirables.
  • Assurer la surveillance pendant et après l’administration des médicaments.
  • Respecter toutes les politiques et procédures d’INVIVA.
  • Déclarer les effets indésirables conformément aux exigences du fabricant, de Santé Canada et aux politiques et procédures d’INVIVA.
  • Élaborer des plans de soins pour les patients.
  • Favoriser l’autonomie des patients pour l’auto‑injection, lorsque requis, en répondant à leurs questions et en leur enseignant à mieux comprendre leur condition et leur médication.
  • Documenter les services de soins aux patients et gérer les dossiers conformément aux politiques et procédures d’INVIVA.
  • Maintenir ses connaissances à jour sur les thérapies médicales conformément aux monographies des produits.
  • Participer aux audits des médicaments et des cliniques, au besoin.
  • Participer activement à la formation et au mentorat des nouveaux membres du personnel clinique.
  • Compléter les formations requises par INVIVA sur les produits et services, les politiques et procédures, la technologie, l’assurance qualité et les communications.
  • Maintenir des normes élevées de qualité, de sécurité et de contrôle des infections afin d’assurer une pratique infirmière sécuritaire.

  • Assurer la continuité des soins entre les équipes infirmières en documentant et en communiquant les interventions, les irrégularités et les besoins en cours.
  • Maintenir et protéger la confidentialité des renseignements des patients.
  • Maintenir un environnement de travail sécuritaire et propre en respectant les procédures, règles et règlements établis.
  • Collaborer avec la direction afin d’assurer le bon fonctionnement de l’équipement en respectant les exigences d’entretien préventif.
  • Maintenir un niveau adéquat de fournitures de bureau.
  • Assurer la disponibilité des fournitures médicales en fonction du volume de patients.
  • Maintenir à jour ses connaissances professionnelles et techniques.
  • Entretenir des relations de collaboration avec les équipes de soins de santé en communiquant efficacement et en répondant aux demandes.
  • Contribuer aux efforts d’équipe et à l’atteinte des objectifs organisationnels.

Formation exigée

  • Infirmier(ère) autorisé(e) / infirmier(ère) clinicien(ne) en règle auprès de l’ordre professionnel provincial
  • Baccalauréat en sciences infirmières ou diplôme d’études collégiales en soins infirmiers (2 ans)
  • Minimum de 3 ans d’expérience en soins critiques (ex. : urgence, soins intensifs) — un atout
  • Expérience en gastroentérologie, rhumatologie, hématologie, oncologie ou neurologie — un atout

Exigences

  • Excellentes compétences en initiation de voies intraveineuses
  • Certification RCR valide
  • Certification ACLS/PALS — un atout
  • Capacité à travailler de façon autonome et efficace, particulièrement en situation d’urgence
  • Aisance à travailler dans un environnement informatisé (ordinateur, MS Office, Teams, WebEx, courriel, dossiers médicaux électroniques)
  • Maintenir annuellement son droit de pratique auprès de l’ordre professionnel provincial
  • Capacité à travailler dans différents lieux selon la région
  • Disponibilité pour travailler le soir et les fins de semaine, au besoin
  • Maintenir annuellement une assurance responsabilité professionnelle (candidats de l’Ontario seulement)

Job summary

Position is responsible for providing professional nursing care to individuals including the intravenous or injection administration of specialty pharmaceutical products. The nurse will help to promote patient health by completing health assessments prior to treatment, reviews patient files and collaborate with physicians and multidisciplinary team members as required. The end goal is to provide medical therapies or self-administration teaching to patients in clinic, home or virtual setting.

Specific responsibilities

  • Review patient files to understand patient’s history.
  • Assess the health status of patients using a pre-screening process that covers contraindications, objective and subjective data, to determine eligibility in receiving medical therapies.
  • Handle and reconstitute specialty pharmaceuticals as per manufacturer product monograph and Heath Canada guidelines.
  • Follow cold chain requirements for medication as per INVIVA policies and procedures and the manufacturers recommendation.
  • As required, insertion of peripheral intravenous or establishing access via central lines based on best practice standards.
  • Follow patient specific medical directives for administration and treatment of adverse events.
  • Provide monitoring during and post medication administration.
  • Follow all INVIVA policies and procedures.
  • Report adverse events in accordance with manufacturer specific requirements, Health Canada and INVIVA policies and procedures.
  • Develop patient care plan.
  • Promote patient independence for self-injection when required by answering questions and teaching patients to understand their condition and medication.
  • Document patient care services and managing records in accordance with INVIVA policies and procedures.
  • Maintain knowledge of medical therapies as per manufacturer’s Product Monograph.
  • Participate in drug and clinic audits upon request.
  • Actively participate in training and mentoring of new clinic staff.
  • Complete training as required by INVIVA on products and services, policies and procedures, technology, quality assurance and communications.
  • Maintain quality, safety and infection control standards to ensure safe nursing practice.

Formal education

  • Licensed Registered Nurse in good standing with provincial college
  • Bachelor of Science in Nursing or 2-year Nursing Diploma
  • 3 + years of critical care experience (i.e. ER, ICU) is an asset
  • Experience in Gastroenterology, Rheumatology, Hematology, Oncology and Neurology is an asset

Requirements

  • Strong intravenous initiation skills
  • CPR certification
  • ACLS/PALS certification is an asset
