Job description

Your Opportunity:

As a Transition Services Case Manager you will be part of the Transition Services Intensive Case Management (ICM) Team based in community. The scope of this team is to facilitate and manage barriers to challenging placements from community and facility and early identification of complex clients to facilitate the most appropriate care stream/specialty program referrals. The Community Transition Services RN ICM is responsible for collaborating with acute care, facility and community partners and stakeholders to promote in-patient client care-planning that mimics community settings The RN ICM advocates for and balances the needs of the individual/family/community with organizational, legislative, and AHS financial accountabilities. This role will require sound critical thinking skills, change management, creative service delivery approaches, and balancing economic, system and ethical decisions in the clinical/home, supportive living (SL), long term care (LTC) and hospital setting. The RN ICM will be based in community, with travel within the city of Calgary to stakeholders in the community, SL and LTC settings pre and post discharge from acute care. The case manager develops partnerships in community, allied health and care providers from Home Care/SL and LTC. The ICM will follow clients up to 30 days once transitioned into care. This role requires access to a vehicle. The ICM will have strong skills in systems thinking, interpersonal communication, conflict management and flexibility. This role is mobile and requires the use of a laptop computer and mobile phone in the community.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $ to $ per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article or (ii), as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): NextGenU Health Assessment Resources Certification:

Additional Required Qualifications: Preferred Qualifications:

Excellent conflict management and stakeholder management skills. Previous experience partnering with mental health, allied health and external partners (SL and LTC). Understands and is able to identify standards and philosophical principles for Case Management. Experience with RAI-HC, PARIS, Strata Health pathways and Sunrise Clinical Manager Electronic Health record.
