THE

ROLE IN A NUTSHELL

Key Responsibilities

• Providing consultation, health teaching and referral to support improved client outcomes within our programs.
• Providing virtual nursing assessments through the Collaborative Care model; these include nursing recommendations and interventions.
• Assisting with the program development, implementation and delivery of Health Services. This includes assess and refer, health teaching, policy implementation recruitment of practitioners, and client follow up.
• Supporting the Nursing team as the program expands / evolves, assisting with the creation of process flows, forms, and service standards.
• Assisting with specialized training for the practitioners providing care within these programs and supporting those clinicians in the expectations surrounding treatment and reporting.
• Being responsible for scheduling assessment / treatment sessions, documenting all interactions with all stakeholders, and providing appropriate reporting updates as required by Inkblot service standards.
• Monitoring compliance, progress, attendance and engagement of all clients engaged in the treatment programs.
• Reviewing all reports received and providing timely reporting to the referring source.
• Development of training materials, patient resources and webinars.
