Position Summary

Clinical Coordinator, General - Palliative Care

Your goal in this position is to provide palliative care mentorship, education, and coaching to health care teams within North York Region.

You will establish relationships with community partners, organizations, and physicians. In addition, you will be responsible for establishing the role of the outreach teams as an effective and efficient interprofessional Model of Care within the healthcare system.

You demonstrate leadership, change management, and problem-solving skills. You have excellent organizational skills and the ability to prioritize workload.

You also have excellent communication skills and demonstrate strong conflict resolution skills. This position supports the hospital’s academic mandate by advancing care through teaching and learning and / or transforming care through research and innovation.

This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for patients, staff, physicians, learners, volunteers and families.

Specifically, Clinical Coordinator will drive practice change, with a focus on improving access to and quality of clinical care, aligned to the Palliative Care Health Services Delivery Framework in the Community ("The Delivery Framework") and the Ontario Palliative Care Competency Framework.

Provide palliative care coaching and mentoring to staff and clinicians in participating community organizations, to build primary-level palliative care competencies.

Collaborate with leaders of community organizations on local service integration, and change management initiatives, using quality improvement methodologies to achieve results.

Consult on clinical care when the complexity of the patients’ needs is beyond the existing competencies of providers (in the community organizations).

Provide support to Indigenous communities and Indigenous organizations for their implementation efforts, where Indigenous communities wish to be involved, in collaboration with regional partners.

On a practical level, you will

  • In partnership with clinical leadership you will take a leadership role in co-creating the regional Delivery Framework Implementation plan with local Ontario Health Team (OHT) and assists with Outreach program development and future direction.
  • Foster regional collaboration by being an active participant in health system transformation to improve care for those in need of palliative care.
  • Act as a resource to bridge local and organizational relationships and inform quality improvement opportunities.
  • Provide a point of contact to influence and create opportunities for system level improvement related to Palliative Care
  • Transfer knowledge by developing, delivering, and coordinating educational sessions to facilitate best practice and practice improvements within palliative care
  • Guide palliative care quality improvement and practice change initiatives
  • Develops and coordinates delivery of education sessions
  • Disseminates learning and information gained across teams to share best practice and inform others about current and future developments
  • Collaborates with internal and external stakeholders in providing community education sessions for hospital, primary care and community care providers.
  • Travel between sites
  • Responsible for project management and coordination of unit and corporate based initiatives.

Qualifications

  • Regulated healthcare professional
  • Current registration in good standing with applicable Health Care Regulated Professional College
  • Minimum 5 years experience in palliative care
  • Minimum 2 years experience in adult learning techniques and knowledge to practice principles.
  • Experience working with quality improvement initiatives
  • Strong clinical coaching and debriefing skills
  • Strong consultation skills, with expert knowledge of models of consultation and their application with individual, team and community service providers.
  • Master’s degree in health care related field or equivalent, preferred.

This is a Part Time Temporary Position of 24 months with 8 hour shifts, Days and Evening shifts in the Palliative Care Unit.

We offer a highly competitive total compensation package which includes pay in lieu of benefits, vacation pay and the opportunity to join the Hospital Pension Plan.
