Job Summary :
Baycrest Hospital Pharmacy Services support all inpatient units and programs. Some of the major functions of the department include : purchasing and inventory management of medications, management of the unit dose drug distribution system, centralized intravenous admixture program, an integrated computerized information system (Meditech), clinical activities such as medication reconciliation and a strong affiliation with the universities and colleges for the provision of onsite practical training.
Pharmacists are active team members focused on the optimization of medication management, who participate in team rounds and quarterly medication reviews.
Responsibilities include but are not limited to :
Participates in relevant rounds and care conferences to ensure safe and appropriate prescribing decisions are made, patient outcomes are monitored and adverse drug events are prevented
Identifies, prevents and resolves drug-related problems by conducting medication reviews, and developing and monitoring patient care plans.
Communicates patient care plans to the interprofessional team, patient, family and / or Substitute Decision Maker
Ensures the seamlessness of continuum of care by conducting medication reconciliation when patients are transferred to or discharged from Baycrest
Ensures appropriate and accurate preparation and dispensing of medications to designated Hospital programs
Participates in departmental medication management projects and Pharmacy clinical programs
- Provides education to Pharmacy students as per the requirements of the Faculties of Pharmacy (University of Toronto and University of Waterloo)
- Provides oversight and direction for Pharmacy Technician(s)
- Ensures best practices are implemented and maintained to enhance medication safety and patient safety
Qualifications include but are not limited to :
- Current and valid license to practice as a Pharmacist from the Ontario College of Pharmacists (Part A)
- Minimum of 2 years of hospital pharmacy experience
- Hospital Pharmacy Residency from an accredited institution or Doctor of Pharmacy Degree (completed or in progress of completing within the year required)
- Demonstrated expertise in the provision of pharmaceutical care
- Self-motivation, excellent communication and organization skills required
- Computer skills that include the ability to work with Microsoft Office Software including Excel, Word & Outlook
- Experience with Meditech preferred
- Well-developed critical appraisal, critical thinking skills and an approach to work that is detail oriented and analytical
- Demonstrated ability to work as an effective member of the interdisciplinary team
