Intensive Case Management
Our client has been providingsupportive housing and shelter for over 35 years with a focus onthe chronically homeless people with complex mental health andaddiction issues and seniors.
Ourclients operate from a harm reduction and clientcentric approach asis looking for a dedicated Intensive Case ManagementWorker.
Theposition :
- Support clientsin their housing by assessing and supporting theirneeds
- Facilitate tenant movementfrom shelters to secure longterm permanenthousing
- Assist tenants inmaintaining housing
- Work with sitestaff to develop outreach and needs assessmentcriteria
- Knowledge of the RTA andHostels Guidelines andProcedures
- Knowledge of currenthousing and support agency providers appropriate to our tenants andresidents
- Ability to understand andassess the suitability of potential landlords and / orresidences
- Identify current issuesdetermine support necessary refer and support clients inpreparation to entrylevel / transitional housing and with thereferring worker support move tosite
- Confer with the frontlinesupport staff at the site to transfer information from the casefile to the staff team
- Utilize casemanagement as a tool to support residents in meeting theirgoals
- Provide support including workreadiness basic life management skills anger management addictioncounseling assertive communication setting goals giving andreceiving feedback selfesteem conflict resolution problemsolvingand managing change
- Provide supportto clients who are facing issues of poverty mental healthaddictions abuse social isolation andmore
- Apply knowledge of groupprocesses anddynamics.
Requirements
Yourskills and experience :
- Youwill have a minimum of 12 years of experience working with peoplewho are homeless underhoused or hard tohouse
- Posses a postsecondary degreeor diploma in social work or an equivalent combination of educationtraining and experience.
- Knowledgeof tenant population diversity issues antidiscrimination practicesand systemic reasons forhomelessness
- Knowledge of relevantlaws standards and guidelines resources and services available inthecommunity
