Pay Range
RPN) $22.44 - $28.99 / (RN) $28.69 - $36.71
The UHN IC program serves clients across diverse specialty populations, necessitating the provision of intricate care and specialized nursing skills within the comfort of their homes.
Specifically designed to cater to individuals discharged from UHN hospitals, nurses in the program deliver personalized nursing care tailored to the unique needs of each client.
A Valid driver’s license and vehicle are required for this visiting nurse role.
UHN visiting nurses are mandated to acquire and maintain a proficiency in specialized clinical skills essential for catering to the diverse and complex needs of clients from UHN.
These skills encompass a range of clinical procedures, including but not limited to pneumostat care, atrium dry seal care, T-tube care, IV Lasix administration, and other specialized medical interventions.
In addition, other comprehensive suites of services include wound care, intravenous (IV) therapy, administering injections, and the removal of PICC lines and chemo.
This multifaceted skill set underscores the nurses' capability to deliver a high standard of care encompassing various medical needs within the home care context.
Key Areas of Accountability
VHA's Nursess are required to do the following tasks, but not limited to :
Assess, Monitor, and Makes Care Planning / Care coordination
Evaluates care plan interventions through on-going reassessment and evaluation results.
Ensures the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
Demostrates professionalism, leadership, judement, and accountability in interactions with client and families.
Supports client safety care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
Informs nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs / submits appropriate documentation to the case coordinator.
Maintains documentation standards according to the College of Nurses of Ontario.
Required Qualifications :
Registered and in good standing with the College of Nurses of Ontario (CNO).
Membership of your Professional Association preferred (RNAO / WeRPN).
RN diploma / degree or RPN diploma from a recognized university or college.
Hold a valid First Aid or CPR Certification.
A moderate level of wound care or venipuncture knowledge and experience is an asset.
Acute Care and Medical-Surgical nursing experience is an asset.
Knowledge of community nursing is an asset.
Demonstrate effective communication (written and verbal) and documentation skills.
Demonstrate reliable attendance, punctuality, and availability for the role.
Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is required.
