Duties
- Work collaboratively, cooperatively and effectively within the Outreach team and with community partner agencies such as, Inner City Health, Shelters, Rooming homes, Drop-ins, Community Health Centers and other hospitals.
- Complete risk assessment and crisis intervention plans, onsite in the community and at a variety of agencies including shelters for the homeless.
- Provide timely and comprehensive written bio-psycho-social assessments, treatment and discharge reports.
- Provide educational opportunities to community partner agency staff regarding mental illness, treatment and recovery.
- Provide short term counselling, education and follow-up to clients.
- Demonstrate thorough knowledge of the theories and practices of a professional social worker.
- Keep up to date regarding professional standards by reviewing current literature, attending education events, networking with colleagues, etc.
- Complete and maintain all requirements for registration with OCSWSSW.
- Attend Social Work peer consultation groups, and participate in peer consultation processes.
- Attend Social Work meetings and participate in discipline specific committees.
- Participate in the teaching, education and supervision of Social Work students.
- Keep current of hospital policies and procedures and applicable legislation.
- Maintain workload measurement and data collection as required (. RAI)
- Perform other duties as assigned.
- Work in a manner that is in compliance with staff and patient / client safety practices, policies and procedures of The Royal.

Qualifications :
- Master’s degree in Social Work.
- Member in good standing with the Ontario College of Social Workers & Social Service Workers.
- Minimum of two (2) to three (3) years of clinical Social Work experience working in a mental health and substance use setting.
- Training and experience in crisis intervention, assessment, consultation and addictions in a community based setting.
- Knowledge of applicable legislated Acts (. MHA, PHIPA, The Health Care Consent Act).
- Familiarity with community resources.
- Collaborative approach to working with community partners.
- Strong conflict resolution abilities.
- Excellent advocacy skills.
- Proven presentation, training and group facilitation skills.
- Demonstrated ability to work independently.
- Effective team skills.
- High level of knowledge, skill and confidence in using electronic health record systems for clinical documentation and navigation.
- Valid Driver’s license and access to a reliable vehicle.
- Bilingual (French / English) is mandatory.English level A- is mandatory in oral expression / comprehension, reading comprehension and written expression.
French level A- is mandatory in oral expression and oral comprehension.
Salary Range : 44.67 to 57.90 per hour
