Your Opportunity :

We are currently recruiting for a full-time Social Worker to join our Interprofessional Primary Care team. This innovative role will be split into two distinct roles, supporting the HFHT’s efforts to expand team-based care in Hamilton and the Greater Hamilton Health Network (GHHN).

This role will provide clinical support to unaffiliated physicians (patients and providers who do not yet have access to team-based care) as part of an interprofessional team playing an integral role in the Hamilton Family Health Team focusing on systems and environmental factors impacting patients' health.

The role will also support the expansion of HFHT treatment and clinical psychoeducation groups.

A Successful Candidate in This Role Will Have :

  • Master of Social Work.
  • Membership in good standing with the Ontario College of Social Work and Social Service Workers.
  • Possess professional liability insurance.
  • Experience in health care settings delivering social work and mental health services.
  • Experience working with multiple partners in Hamilton or other communities.
  • Experience, expertise and additional training in intervening on social determinants to health and mental health.
  • Experience working in health care and mental health settings on an interprofessional team.
  • Health promotion experience preferred.
  • A strong understanding of applying legislation in practice (PHIPA, etc.)
  • A strong understanding of the application of Social Work Values and Ethics in clinical practice with a team in a health care setting.

Duties and Responsibilities :

  • Perform biopsychosocial assessments and formulate and deliver treatment interventions.
  • Provide short-term treatment, with evaluation and resource referrals where required.
  • Monitor and adapt treatment collaboratively with patients, their primary care provider and interprofessional team.
  • Participate in the development and execution of health promotion activities to increase outreach and patient enrollment.
  • Assist patients and providers in navigating the health and community service systems to identify and connect them with appropriate services and supports.
  • Participate in team building, program development, and quality improvement activities.
  • Advocate with patients and their families in matters affecting health.
  • Liaise and build relationships with other primary care providers in the health care system and other providers and agencies in the community as required.
  • Teach family medicine residents and medical and allied health students about the role of the social worker and psychosocial issues facing patients and families as required.
  • Provide communication to hospital and community staff and other interprofessional team members as required to ensure continuity of care.
  • Some case management, including links to community services in areas such as assistance with daily living, crisis intervention, consulting with MAiD, and health promotion and prevention.
  • Support patients in achieving goals and improving functioning in community integration, health and wellness, employment, financial literacy, personal care, household management, housing, and food security.
  • Provide evidence-informed mental health interventions with patients and families.
  • Develop and facilitate evidence-informed treatment groups and patient education groups collaboratively with your interprofessional team and clinical leadership to meet patient and system needs.
  • Community connection, outreach, and partnership building.
  • Co-design processes to attach new unaffiliated physicians and their patients to these services.
  • Collect data as required for statistical / reporting purposes.
  • Provide informative reports as directed and requested by the Manager, Primary Healthcare or Professional Practice Lead.
  • Participate in staff meetings and ongoing professional development opportunities as required.
  • When required, assist in developing and implementing the clinical model and guidelines for collaborative care.
  • Outreach to the community.
  • Understanding the needs of the broader community and the HFHT patients.
  • Group facilitation (virtual and in-person).
  • Group evaluation.

The above responsibilities are not to be considered all-inclusive.

Location(s) : The home base for the position will be the Hamilton Family Health Team Central Hub. The incumbent will be required to attend partner sites regularly.

Summary

Classification : Union (CUPE)

Employee Class : Full-Time; 40hrs / week

Schedule : Monday-Friday; Evening hours as required for group facilitation

Salary : $74,148
