Job Description
Functions as an active member of a multidisciplinary team in the provision of patient-focused care for the Rehab / Geriatric Unit.
The successful candidate will collaborate with other health care professionals in the planning and implementation of care for a wide variety of Rehab / Geriatric cases.

- Current Registration with the College of Nurses of Ontario.
- Current Heart & Stroke approved Basic Life Support (BLS) certification required at time of hire
- Medication administration and asceptic technique certification mandatory.
- 1-2 years experience in the Rehab / Geriatric setting.
- Strong geriatric / medical / surgical / background.
- Advanced RPN assessment skills ( catheterization care, skin / wound care).
- Caring and compassionate attitude towards patients and families.
- Able to work effectively in a multidisciplinary Rehab team setting
- Ability to co-ordinate and interface with community resources, CCAC and family members, discharge conferences.
- Must be able to meet the physical demands of the position such as lifting and transfer of heavy care patients, utilizing assistance of others / equipment as required.
- Satisfactory employment and attendance record required.
The ideal candidate is strong in patient centered care in a dynamic, rapidly changing environment and should be ready to drive change and innovation.
We require a person with good communication skills with a clear patient focus with the ability to manage execution and to foster teamwork and cooperation.

Hours : Monday to Sunday - Days and Nights 12 hour rotating shifts 0730-1930, 1930-0730 (hours subject to change based on operational requirements)
