Community Physiotherapist – Community Advantage Rehabilitation (CAR) – a division of Lifemark.

We are currently seeking qualified Registered Physiotherapists to provide services to children within the community, therefore current or prior experience of working on pediatric cases, is required. This position is a flexible role, and the working hours range from part-time to full time depending on your desired client caseload size.

PT Responsibilities :
Provide ministry funded, community based physiotherapy services within the client’s place of residencePerform physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of geriatrics, neurology, orthopedics, palliative careDevelop and implement goal-oriented treatment plans in collaboration with the client / caregiversAssign patient care plan, and provide supervision and monitoring of physiotherapy assistants when appropriateProvide education to client / caregivers and link to community agencies / resources when appropriateComplete and maintain documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standardsRequirements :
Registered Physiotherapist with the College of Physiotherapists of Ontario (required)Experience in working with in a community setting is preferred but will consider new graduatesAbility to work independently and as part of an inter-professional teamAbility to prioritize, manage self-scheduling and meet reporting deadlinesExcellent oral, written skills and report writingProficiency with computersStrong interpersonal and organizational skillsReliable transportation is required for this positionLifemark welcomes and encourages applications from people with disabilities.
