Job Description

GEM Nurse Emergency

As the Geriatric Emergency Management (GEM) Nurse, you’ll provide comprehensive, specialized and advanced nursing care for patients in the emergency department.

The candidate will practice within the inter-professional team model and be responsible for promoting best practices in care of older adults and for excellence in the nursing care of older patients in acute care.

The successful candidate will liaise with other hospital, community, long-term care services in York Region and, other GEM nurses in the city and across the province.

Contribute to safe, quality care by

Sharing specialized expert knowledge through the consultation process and may focus on clinical issues relevant to : structure, (environment, delivery of care);

process (developing nursing standards, program development); or outcomes.

  • Providing consultation support and management recommendations for the care of older adults in the ED.
  • Obtaining all relevant information to assess, analyze, plan, implement and evaluate a plan of care / action to coordinate identified client needs using standardized geriatric assessment tools.
  • Demonstrating collaborative practice as an integral part of the inter-professional team.
  • Facilitating application of advanced practice within the organization.
  • Evaluating clinical outcomes and recommends changes in practice to improve the quality of care.
  • Identifying recurring clinical issues / problems and communicates findings / suggestions for change appropriately.
  • Initiating and maintaining links with other GEM Nurses in York Region to advance clinical outcomes for older adults receiving care in the ED.
  • Analysing and interpreting best evidence related to care of older adults.
  • Designing & providing educational and learning opportunities for staff and acts as a resource for related issues and problem solving.
  • Participating in the development of public and professional programs and learning resources.
  • Contributing to the quality of nursing knowledge and practice through a variety of research related activities.
  • Disseminating and facilitating the implementation of recent innovations and research findings relevant to nursing practice and client outcomes.
  • Evaluating the present practice at the individual and system levels in light of current research findings.
  • Assigning appropriate activities to other staff and volunteers, provides effective supervision, leadership and direction.
  • Managing a workload effectively in a fast-paced environment.
  • Addressing and resolving issues in a productive, respectful manner.
  • Exercising sound professional judgment and using best practice guidelines.
  • Contributing to a safe, inclusive environment for all through compliance with patient and staff safety policies and procedures.

What must you have?

  • Baccalaureate degree in Nursing from an approved nursing program.
  • Current Certificate of Registration in the General Class with the College of Nurses of Ontario in good standing.
  • Current certificate of competence from the College of Nurses of Ontario as an RN or RN (EC).
  • Minimum of five (5) years of demonstrated current expertise in geriatrics and / or emergency services.
  • Membership in the Registered Nurses’ Association of Ontario.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Advanced Cardiovascular Life Support (ACLS) or commitment to completing within 1 year of hire.
  • Proven clinical experience with patient-focused care, including strong patient assessment, care planning, patient education and documentation skills.

  • Experience working with electronic medical records.
  • Advanced clinical knowledge, program design and evaluation, project management and implementation of evidenced based practice.
  • Working knowledge of the College of Nurses of Ontario standards of practice.
  • Superior interpersonal, coaching, and leadership skills to facilitate identification and development of staff learning needs.
  • Facilitates application of relevant information to make effective judgments and decisions to lead the management of care.
  • Superior assessment skills.
  • Working knowledge of the Regulated Health Profession Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act.
  • Demonstrated experience and proficiency in the provision of effective clinical consultation for the complex frail elderly.
  • Evidence of program design and resource development skills.
  • Excellent practice program development and project management skills. Self-directed, works independently and able to effectively lead project teams to achieve outcomes.
  • Effective communication skills, engages others, and is a reflective listener and learner.
  • Proficient with computer programs including; Word, Excel, PowerPoint, & Outlook.
  • Excellent report writing skills.
  • Proven analytical thinking and proactive problem solving and decision making skills.
  • Familiarity with the Regulated Health Professions Act, Occupational Health and Safety Act, WHMIS, and the Standards of Practice of the College of Nurses of Ontario.
  • Excellent interpersonal, listening, and verbal and written communication skills in English, with an ability to deliver information and work effectively with patients, families and colleagues.
  • Demonstrated strong decision-making, problem-solving and critical thinking skills.
  • Demonstrated ability to take initiative and adjust to new or unexpected events.
  • Demonstrated commitment to continuous learning and professional development.

Preferably, your profile also includes :

  • Master’s Degree in Nursing
  • Gerontological Nursing Certification.
  • Speciality certifications related to the area of expertise.
  • Recent completion of a physical assessment course.

Ability to speak a second language.
