Description
The Health Information Management (HIM) Professional serves as the subject matter expert in Health Information Management support. The HIM Professional provides advanced level operational support in ensuring the integrity of the Electronic Health Record (EHR) and is responsible for ensuring and overseeing the delivery of complete and accurate clinical records in full compliance with provincial legislation, Accreditation Canada, standards of practice and information management processes.
Major Responsibilities
- Specialized handling and managing of complex and sensitive requests for patient health information, including ensuring compliance with legal and regulatory standards and serving as the primary point of contact for external agencies, legal entities, and patients regarding ROI inquiries.
- Act as delegated Officer-in-Charge to oversee mental health act forms requirements per the organizational policies and procedures.
- Work with the director of Health Information to support the hospital wide health information governance structure.
Coordinate, monitor and maintain the Health Information Data Governance structure, including but not limited to :
Facilitate chart audits and local ROI as it relates to data integrity of scanned documentation to ensure accuracy, privacy and efficiency.Develop and / or manage key performance indicators and quality improvement initiatives.Support reporting requirements and validations.Training and Guidance : Provide training and guidance to other staff members, particularly unit secretaries, on routine ROI processes and compliance issues.Codes and abstracts sensitive data for mandated provincial reporting requirements from patient charts while ensuring accuracy of data.Performs lead support for the Consent and Capacity Board hearings, including receiving and processing all applications to the Board for a hearing, liaising, arranging and organizing the hearing with the attending physician, patient advocate, Rights Advisor, unit staff, family and legal representation and the Board.Other Duties
Resolves / clarifies conflicting, missing or unclear information with physicians.Interdepartmental Coordination : Facilitate communication between various departments, ensuring timely and accurate release of information.Participates with data quality improvement initiatives.Responsible to recognize and report patient, staff, and environment safety issues, identify and implement preventative and corrective actions, within the scope of this role.Performs a quantitative / qualitative analysis of the clinical record for completeness, accuracy, validity to ensure information meets hospital accreditation standards, statutory and legal requirements.Verification, oversight and quality assurance of documents scanned in the electronic medical record system by unit secretaries.Completes chart audits as required.Other duties as required by the Director / Manager of the Service.Knowledge
Bachelor’s Degree (preferred) or College diploma (required) in Health Information Management or related field.Certification with and member in ‘good standing’ with Canadian Health Information Management Association (CHIMA) required.Minimum four (4) years health records knowledge and experience preferred.Minimum four (4) years of recent Release of Information and Delegated Officer-in-Charge duties preferred.English level A- is mandatory in oral expression, oral comprehension, reading and writing. Bilingual (French / English) is considered an asset.Experience in a hospital and / or mental health environment preferred.Knowledge of Personal Health Information Protection Act, the Mental Health Act, Substitute Decisions Act, Criminal Code of Canada and adherence to appropriate legislation with respect to Privacy and Confidentiality.Experience using Meditech Expanse 2.1 an asset.Skills
Strong organizational skills with demonstrated ability to prioritize in a fast-paced environment.Advanced understanding of technology and how it can be applied in a clinical environment.In depth knowledge of ICD-10, DSM IV coding classification system.Knowledge of medical terminology.Knowledge of MIS guidelines, OHRS (Ontario Healthcare Reporting Standards), DAD, NACRS, NRS and RAI methodology (Inter-RAI) preferred.Strong keyboarding and computer skills with attention to detail and accuracy.Demonstrated presentation, meeting and workgroup facilitation skills.
Ability to apply analytical skills for problem identification, understanding and mapping solutions.Proven ability to foster good public relations and effective working relationships within the team and hospital.High level of proficiency in Microsoft Office Suite Applications.Knowledge of terminal digit filing.Proven excellent interpersonal skills and effective oral / written communication skills.Demonstrated excellent organizational, analytical, time management, prioritization, problem-solving, and conflict resolution skills.Demonstrated ability to work under pressure with minimal supervision.Proven good attendance record with the capability of maintaining this same standard.Ability to perform the physical demands of the position.Other Information
