    Fonction de base

    Le ou la spécialiste des soins aux patients (SSP) est le point de contact unique pour les patients inscrits au programme de soutien aux patients. Il incombe au ou à la spécialiste des soins aux patients de s’assurer que les offres de services des programmes sont gérées et livrées efficacement avec un service à la clientèle de la plus haute qualité. Ce ou cette collègue est axé(e) sur l’action, fait preuve de diplomatie et de compréhension, et s’assure que les patients tirent une expérience positive du Réseau de santé spécialisé durant leur traitement.

    Nature et portée

    Le ou la spécialiste des soins aux patients est le point de contact principal pour les patients en plus d’être, dans certains programmes, le point de contact unique pour le groupe de cliniques et de bureaux de ce territoire. Le ou la spécialiste des soins aux patients collaborera avec d’autres spécialistes des soins aux patients et un(e) gestionnaire de cas ou gestionnaire de cas sur le terrain pour le territoire désigné, selon le programme. Le soutien supérieur offert de bout en bout par cette équipe améliorera l’expérience des patients et des médecins avec le programme de soutien et favorisera des résultats positifs pour les patients. L’attention portée aux indicateurs de rendement clé des clients ciblés pour le programme est indispensable pour ce poste.

    Tâches :

    • Agir comme point de contact unique des patients inscrits au programme de soutien et des médecins prescripteurs, le cas échéant
    • Effectuer les appels de bienvenue pour recueillir des renseignements pertinents, présenter le programme de soutien aux patients et obtenir le consentement des patients, au besoin
    • Informer les patients sur l’ensemble du programme; par exemple, soutien à la gestion de cas, soutien au remboursement, formation sur l’auto-injection, coordination des ordonnances et conformité continue
    • Comprendre très bien le médicament
    • Maintenir un niveau de connaissance élevé des mécanismes de remboursement des secteurs public et privé, plus précisément ceux du territoire assigné
    • Défendre les intérêts des patients, explorer toutes les options de financement des médicaments, déterminer les détails de la couverture, et élaborer et tenir à jour la documentation, complète et exacte, requise pour favoriser la réussite du processus de remboursement
    • Administrer les essais financiers et gérer tous les aspects de l’aide financière du programme, le cas échéant
    • Assurer la coordination des services aux patients du programme de manière efficace pour un accès rapide et continu au traitement, y compris la formation sur l’auto-injection
    • Travailler en collaboration avec la gestion du programme, d’autres spécialistes des soins aux patients et le ou la gestionnaire de cas en magasin ou gestionnaire de cas (selon le programme) pour répondre aux préoccupations ou aux besoins des patients et pour traiter les recours hiérarchiques qui interrompent le cheminement des patients
    • Communiquer avec les clients périodiquement pour confirmer l’adhésion et coordonner la livraison des médicaments (y compris la date et l’heure)
    • Signaler immédiatement ou dès que possible tous les événements indésirables (EI) et plaintes techniques sur le produit (PTC), conformément aux politiques et procédures en vigueur
    • Qualifications :

    • Diplôme d’études collégiales ou universitaires requis
    • 2 à 3 années d’expérience dans les secteurs pharmaceutiques ou des soins de santé, et de préférence avoir travaillé directement avec les patients et d’autres professionnels de la santé (p. ex., médecins, personnel infirmier, pharmaciens ou pharmaciennes)
    • Expérience avec les régimes d’assurance-médicaments publics (provincial et fédéral) et privés de tiers
    • 2 à 3 années d’expérience en soutien à la clientèle ou dans un centre d’appels
    • Compétences solides en informatique dans un environnement Windows avec MS Office et expérience avec une base de données de relations avec les clients, un atout
    • Bilinguisme anglais et français requis
    • Capacité avérée à travailler dans un environnement dynamique et en équipe
    • Excellentes aptitudes en communication, verbale et écrite, pour interagir efficacement avec les patients et les clients
    • Capacité avérée à analyser et à interpréter les renseignements, les données et les règlements
    • Grand sens de l’attention aux détails et capacité avérée à prendre des décisions éclairées
    • Flexibilité pour travailler de longues journées et des heures supplémentaires, au besoin
    • Être en mesure de travailler selon un horaire flexible entre 8 h et 20 h.

      Basic Function

      The Bilingual Patient Care Specialist (PCS) is the single point of contact for patients enrolled in a patient support program. The PCS is responsible for ensuring program service offerings are managed and delivered effectively with the highest quality customer service. This colleague is action oriented, tactful, and empathetic, ensuring the patient has a positive experience working with Specialty Health Network through their treatment journey.

      Nature and Scope

      The Bilingual Patient Care Specialist is the main point of contact for the patient, and in some programs, also the single point of contact for the group of physician clinics / offices within that territory. The Patient Care Specialist will work collaboratively with other Patient Care Specialists and a Case Manager / Field Case Manager for a designated territory, dependent on the program. The superior end to end support provided by this team will enhance the patient and physician experience of the Support Program and drive positive patient outcomes. Attention to customer key performance indicators identified for the program is fundamental to this role.

      What you will do :

    • Single point of contact for patients enrolled in the support program and prescribing physician, if applicable
    • Conducts welcome calls to gather relevant information, introduces patients to the support program, and obtains patient consent as required
    • Informs the patient about all program offerings – for example case management support, reimbursement support, self-injection training, prescription coordination and ongoing compliance
    • Understands the molecule(s) in full detail
    • Maintains an expert level of knowledge of reimbursement mechanisms in the public and private arenas, specifically within assigned territory
    • Acts as a patient advocate, exploring all options for funding of medications, determines coverage details and develops / maintains complete and accurate documentation required to facilitate a successful outcome of the reimbursement process
    • Administers financial means tests and manages all aspects of financial assistance within the program as applicable
    • Ensures coordination of program services to patients in an effective manner resulting in prompt and continued access to therapy, including self-injection training
    • Works collaboratively with Program Management, other Patient Care Specialists, and Field Case Manager / Case Manager (program dependent) to address patients concerns or needs and escalations interrupting the patient journey
    • Contact clients periodically to confirm adherence and coordinate delivery of medications (including date, time)
    • Report all adverse Events (AE) and Product Technical complaints (PTC) immediately or as soon as possible upon becoming aware of the event as per current policies and procedures
    • What you will need :

    • College Diploma or University degree is required
    • 2-3 years’ experience in the healthcare or pharmaceutical industry, preferably working with directly with patients and with other health professionals (i.e., physicians, nurses, pharmacists)
    • Experience with public (provincial and federal) and private third-party drug reimbursement plans
    • 2-3 years’ experience in a Customer Service or Call Centre setting
    • Strong Computer skills in a Windows environment using MS Office and experience using a customer relationship database is an asset
    • Bilingualism (French & English) - required
    • Demonstrated ability to work successfully within a fast paced, team-based environment
    • Excellent communication skills, written and verbal, to interact effectively with patients and clients
    • Demonstrated ability to analyze and interpret information, data, and regulations.
    • Demonstrated ability to pay attention to details and make decisions using sound judgement.
    • Flexible to work longer hours, overtime as required
    • Able to work flexible hours between 8 : 00AM to 8 : 00PM
