Position Overview

The Registered Dietitian within the Algoma Diabetes Education and Care (ADEC) program is responsible for providing comprehensive nutrition assessment, care planning, and education to patients and families in both individual and group settings. The Dietitian collaborates with the interdisciplinary health care team and community partners to support a patient-centered approach to diabetes management. This role also contributes to program development through participation in quality improvement initiatives, health promotion activities, committees, and the development and evaluation of clinical standards, policies, and educational resources.

Essential Duties & Responsibilities

Clinical Care & Patient Support:

  • Conduct comprehensive nutrition assessments, including medical, social, and dietary history, toi dentify patient needs and risks.
  • Develop, implement, and evaluate individualized nutrition care plans aligned with best practices in diabetes management.
  • Provide evidence-based nutrition counselling and education to patients and families in both individual and group settings.
  • Support patient self-management through education, goal setting, motivational interviewing, and behaviour change strategies.
  • Monitor patient progress and adjust care plans as appropriate.

Interdisciplinary Collaboration:

  • Collaborate effectively with physicians, nurses, diabetes educators, and other members of the interdisciplinary team to ensure coordinated and holistic care.
  • Communicate relevant patient information clearly and professionally within the circle of care.
  • Liaise with external agencies and community resources to support continuity of care.

Program Development & Quality Improvement:

  • Participate in the development, implementation, and evaluation of program initiatives, policies, and clinical standards.
  • Contribute to continuous quality improvement activities, including data collection, outcome measurement, and program evaluation.
  • Support the development and delivery of group education programs and health promotion initiatives.

Education & Health Promotion:

  • Develop and deliver engaging, culturally appropriate education materials and sessions for patients, families, and the community.
  • Promote healthy eating and diabetes prevention/management strategies through outreach and community engagement.
  • Stay current with emerging research, guidelines, and best practices in nutrition and diabetes care.

Professional Practice & Compliance:

  • Maintain accurate, timely, and confidential patient documentation in accordance with organizational policies and regulatory standards.
  • Adhere to the standards of practice set by the College of Dietitians of Ontario and other relevant regulatory bodies.
  • Demonstrate professionalism, ethical practice, and a commitment to ongoing learning and development.
  • Uphold organizational values and contribute to a positive and respectful work environment.

Required Qualifications

Education & Licensure/Certification

  • Registered Dietitian with a degree in Human Nutrition or equivalent, and current registration in good standing with the College of Dietitians of Ontario

Knowledge, Skills, & Abilities

  • Demonstrated knowledge of medical nutrition therapy, with experience working collaboratively with physicians and other health care providers.
  • Proven ability to perform comprehensive nutrition assessments, develop and implement care plans, and deliver effective nutrition education to patients and families.
  • Excellent interpersonal skills, with a flexible, adaptable, and creative approach to problem solving.
  • Demonstrated ability to work effectively as a member of an interdisciplinary team and in collaboration with external agencies, applying sound clinical judgment and decision-making skills.
  • Well-developed self-awareness, emotional intelligence, and the ability to manage complex or sensitive situations with professionalism and empathy.
  • Strong communication skills, both verbal and written, with the ability to convey complex information in a clear, concise, and patient-centered manner.
  • Ability to establish, build, and maintain effective working relationships with patients, families, team members, and community partners.
  • Demonstrated ability to engage, educate, and empower patients to support behaviour change and self-management.
  • Strong organizational and time management skills, with the ability to manage competing priorities in a fast-paced clinical environment.
  • Demonstrated commitment to maintaining patient privacy, confidentiality, and ethical practice standards.
  • Commitment to continuous learning, professional development, and maintaining current knowledge of best practices.
  • Ability to consistently demonstrate GHC’s values of respect, kindness, compassion, and accountability in all aspects of work.

Compensation:

  • $69, – $86,
