Job Description

The Licensed Practical Nurse will provide professional nursing care to residents, including medication administration and mentoring unregulated staff.

Responsibilities :

• Develop care plans under the direction of the Director of Care or designate for physical, psychological and social care of residents.
• Direct and assist care aides in using appropriate procedures to ensure high standard of resident care.
• Administer, monitor and record medication administration, educate resident as needed.
• Create and manage nursing care plans, participate in evaluation of current and future medical needs.
• Evaluate effectiveness of nursing care treatments and inform Health & Wellness Manager or designate and / or resident's physician of significant changes in condition of any resident.
• Provide personal care assistance, nursing procedures, mentorship and coaching of Health Care Aides.
• Work with residents to meet care requirements, communicate concerns to doctor and manager as needed.
• Demonstrate ongoing competence utilizing knowledge, skill, judgment, and evidence-based practice in caring for aging adults.
