Job description

What you’ll be doing :

The Licensed Practical Nurse provides direct health care services to patients presenting with a wide variety of complex addictions and mental health issues. Working within scope of practice, you will perform physical, addiction, and mental status assessments; refer patients to the physician for further examination; and administer prescribed treatments and medications. You will observe, monitor, evaluate, and document patient progress, symptoms, and behavioural changes, and perform a variety of health care interventions. You’ll also provide counselling, education, and support using a motivational and health promotion approach and work collaboratively with other members of the interdisciplinary team, the patient, the family, and the community.

  • Implement the principles and practices of a patient and family centered recovery model in mental health and addictions treatment
  • Conduct comprehensive mental health and addiction risk assessments, formulate treatment and discharge plans, and provide crisis intervention
  • Perform detox assessments
  • Communicate and collaborate with patients and families about health findings, diagnosis, treatment, self-care, and prognosis
  • Assess patients daily for care needs and follow up; consult with doctors and clinical team members about patient care
  • Collaborate with patients and other health professionals to identify and assess trends and patterns that have implications for patients, families, and communities
  • Analyze and synthesize all available information for the development of an individualized treatment plan in collaboration with other care providers, patients, and families
  • Identify and respond appropriately to legal and ethical issues that may arise in patient care.
  • Conduct day-to-day care delivery to patients and administer medications
  • Provide nursing intervention as required and determine when health care needs to be escalated to other health professionals
  • Coordinate referrals to appropriate health resources. Liaise with health practitioners and agencies for the planning and implementation of health care
  • Advocate for the patient
  • Perform appropriate clinical monitoring
  • Evaluate efficacy of care and treatment plans and adjust as necessary
  • Intervene in emergencies, crises, or violent situations, according to the needs of the patient and family
  • Document assessments and patient progress, keep records current and complete documentation on time
  • Mentor and act as a preceptor for students as required

What we are looking for :

  • Licensed Practical Nurse Diploma
  • Current full practicing licensure with the British Columbia College of Nursing Professionals (BCCNP)
  • Completion of Non-violent Crisis Intervention course preferred
  • Current First Aid / CPR Certification for health care providers (Level 1 or greater)
  • Demonstrated ability to deal with a variety of situations and responsibilities requiring initiative, creativity, and professional judgment
  • Ability to work with patients with mental health and substance abuse issues
  • Knowledge of community, social, and mental health resources
  • Ability to effectively manage conflict situations and aggressive behaviour, and use non-violent crisis intervention and behavioural management techniques
