As a Social Services Worker at Norview Lodge, you’ll plan and deliver programs, admissions support, and advocacy services that enhance residents’ quality of life. Your work will provide guidance, emotional support, and ethical leadership, helping residents and families navigate transitions, complex care needs, and end‑of‑life decisions.

Duties and Responsibilities:

  • Oversee new resident admissions in consultation with Supervisors, Nursing and Personal Care.
  • Participation on committees as needed, e.g., Responsive Behaviours and Pain and Palliative Care Committee.
  • Serve as the Chari of the Ethics Committee, facilitating ethical discussions, guiding ethical decision-making, support staff and families, and promoting resident-centered care in situations involving complex care needs, responsive behaviours, and end-of life issues.
  • Provide education and information for staff and family members regarding responsive behaviours, end-of-life care, M.A.I.D. and disease progressions.
  • Facilitate tours for prospective admissions to the home and facilitate Norview Greeter program.
  • Provide emotional support and ongoing assistance to Residents and families regarding placement, family dynamics and end-of-life care.
  • Take on a leadership role in problem solving, program planning and navigating barriers to access appropriate services.
  • Promote a positive atmosphere for Residents and families.
  • Advocate on behalf of the Residents regarding rights, preferences, and quality-of-life.
  • Maintain accurate and timely documentation on all Residents and interventions.
  • Participate in interdisciplinary care conferences for Residents.
  • Facilitate or support family meetings in collaboration with the interdisciplinary team.
  • Provide support for Recreation Therapists related to resident programming.
  • Participate in Continuous Quality Improvement (CQI) initiatives such as Resident and Family surveys.
  • Ongoing education related to social issues and policy.
  • Maintain awareness of and provide referrals to community resources and support services, as appropriate i.e. BSO.
  • Other duties as assigned.

Requirements

Knowledge and Experience:

  • Grade 12 plus a Social Services Worker Certificate from a College of Applied Arts and Technology plus proof of current registration with the Ontario College of Social Workers and Social Service Workers.
  • Recent experience in long-term care an asset.
  • Experience and skill in crisis intervention, transitional care, loss and bereavement support.
  • Experience with computerized documentation systems and interRAI LTCF.

Skills and Abilities:

  • Knowledge of the Fixing Long-Term Care Homes Act, 2021 and Regulations; Personal Health Information Protection Act, 2004.
  • Ability to work independently and efficiently in a dynamic, fast-paced environment.
  • Demonstrated proficiency in communication, resident advocacy, interdisciplinary collaboration, and consultation with care.
  • Excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands.
