Pay Range
OPSEU / VHA Pay Scale 4a : $24.19 per hour
Reporting to the Senior Manager, Client Care and Operations, this position is responsible for perform all intake functions for the Community Support Programs including programs service coordination and support : receive, assess, and process all requests for service from potential clients and referral sources, provide information regarding the criteria for the programs and range of services offered and refer to other community services when ineligible. Participates in VHA services and quality initiatives. All activities are conducted within the policies, procedures and standards of confidentiality of VHA.
Key tasks and responsibilities of the role :
- Conduct initial assessments to understand the needs, challenges, and goals of clients.
- Collect and review relevant client information, including medical, psychosocial, and family histories.
- Assess eligibility for programs and services based on established criteria.
- Provide detailed information about available programs, resources, and services.
- Refer clients to appropriate internal or external services, such as counseling, housing assistance, or healthcare providers.
- Follow up on referrals to ensure clients successfully connect with recommended services.
- Maintain accurate, detailed, and confidential client records.
- Document assessments, case notes, and referrals in compliance with organizational policies and legal requirements.
- Prepare reports and summaries as needed for supervisors or team meetings.
- Work closely with other social workers, healthcare providers, and community organizations to coordinate services for clients.
- Participate in team meetings to discuss case plans and ensure cohesive service delivery.
- Advocate for clients within multidisciplinary teams or community networks.
- Identify gaps in services and provide feedback to improve intake processes and client programs.
- Participate in community outreach to increase awareness of available resources.
- Build and maintain relationships with external service providers and referral sources.
- Provide clients with education on navigating social systems and accessing benefits.
- Offer emotional support and guidance during the intake process to foster trust and rapport.
- This role requires strong communication, problem-solving, and organizational skills, as well as a commitment to empathy and client advocacy.
