Job description

The Service Access and Transitions Leader is the creative relationship-builder responsible for key elements of service access, including move-in and move-out from CONNECT and developing key relationships in the community and within the health region.

The role oversees:

  • Service Access and monitoring, including the implementation of goal attainment scaling;
  • Transitions to and from CONNECT (both publicly funded and privately funded bedrooms); and
  • Exploring other service potential.

This leader will work closely with the Vancouver Island Health Authority and other partners and funders to facilitate smooth flow and transitions into and out of CONNECT. They will also oversee the development of CONNECT’s private capacity, including building the key relationships with private funders and the ABI community to keep an eye on the health of our private capacity. They will oversee the move-out of CONNECT for the people we support and their integration into their home communities.

REPORTS TO: CONNECT Communities’ Director of Culture, People and Services

JOB REQUIREMENTS & COMPETENCIES:

  • Intuitive grasp of who we are and what we are trying to do and ability to communicate this knowledge to others
  • Authentic leadership and ability to build sustainable, productive relationships with the people we support and their families, coaches, leaders, funders and other external stakeholders
  • Experience navigating and building relationships within the health region
  • Experience working with individuals with mental health and substance use
  • At least five years of experience leading, mentoring and coaching people for growth; when necessary, setting clear expectations and monitoring progress towards them
  • Sound planning and problem-solving skills and ability to ‘keep your eye on the donut and not the hole’
  • Sound resource management and financial stewardship
  • Experience with operational planning and implementation

KEY TASKS & RESPONSIBILITIES

  • Liaise with our partners to determine fit and follow-up with the initial creation of the Life Redesign Plan for those moving to CONNECT
  • Act as communication point person between CONNECT Leaders, the coaching resource team and the health region
  • Fulfill other health region obligations
  • Manage inquiries
  • Develop contracts for each individually contracted bedroom
  • Develop an understanding/expertise and relationships with other private funders
  • Create and maintain a meaningful connection to the ABI community on Vancouver Island
