Clinical Planner - RPN page is loaded## Clinical Planner - RPNlocations : ParaMed - Kitchener, Ontariotime type : Full timeposted on : Posted Todayjob requisition id : R29385 Job Description The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions. ACCOUNTABILITIES Performs the initial assessment of care needs for new patients and establishes the individual care plans for patient services, in accordance with Funder service requirements.* Completes the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions.* Performs the initial personal care services for new patients.* Assesses patient needs to identify the appropriateness for either Health Support Worker or Personal Support Worker care.* Assesses the ability to shift scheduled patient care to off peak times, based on clinical need, in order to increase system capacity.* Advises Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.* Informs patients and families of ParaMed policies and scheduling practices, as well as the expected time of care.* Maintains current and accurate patient records.* Shows respect for cultural needs, dignity, privacy, and confidentiality of patients and staff.* Stays abreast of changes in practice setting that affect scope of practice.
