The New Vision Family Health Team (NVFHT)is seeking a full-time (0.8-1.0 FTE), permanent, Registered Nurse.
The NVFHT is a multi-disciplinary team located in the Waterloo Region providing primary care to over 30,000 patients. We offer a friendly, collaborative and professional environment in a spacious facility.
As a key member of the New Vision Family Health Team, the Registered Nurse (RN) provides comprehensive primary care nursing services with a strong focus on chronic disease management, particularly hypertension and diabetes. The RN works collaboratively with physicians, nurses, allied health professionals, and administrative staff, while also functioning autonomously within established medical directives and professional standards. Strong interpersonal and communication skills are essential to effectively engage patients, promote self-management, and optimize health outcomes.
Key Responsibilities :
- Participate in and support chronic disease management programs, with a primary focus on hypertension and diabetes care.
- Conduct comprehensive patient assessments, including vital signs, physical assessments, screening, and data collection.
- Provide ongoing monitoring, education, and follow-up for patients with hypertension and diabetes, supporting treatment plans and lifestyle modification.
- Maintain and manage INR monitoring systems in collaboration with physicians.
- Administer medications and injections (including allergy and other injections) in accordance with College of Nurses of Ontario (CNO) standards, clinic policies, and medical directives.
- Implement medical directives confidently and appropriately within RN scope of practice.
- Communicate test results to patients and coordinate follow-up care as directed by physicians.
- Triage and respond to patient telephone inquiries within scope of practice, escalating concerns as required.
Education and Certification
Bachelor of Science in Nursing (BScN) from an accredited nursing program or recognized equivalent.Current registration in good standing with the College of Nurses of Ontario (RNO).Current membership with the Registered Nurses Association of Ontario (RNAO).Certified Diabetes Educator (CDE) designation or willingness to obtain certificate required.Current Basic Life Support (BLS) / CPR certification.Skills and Competencies :
Demonstrated ability to work both independently and collaboratively within an interdisciplinary primary care team.Strong organizational skills with the ability to prioritize, manage time effectively and adapt to fast-paced clinical environment.Excellent communication and interpersonal skills to support patient education and team collaboration.Demonstrated sensitivity to social, cultural and individual patient needs.Additional Requirements
Clear Criminal Record Check with Vulnerable Sector Screening.Completion of required health screening (MMR, Varicella, Tdap, Hepatitis B).Proof of professional liability insurance.Job Requirements :

Hourly wage range : $42.50 to $43.69Healthcare of Ontario Pension Plan (HOOPP).Extended healthcare plan.Paid time off, including : 8 paid sick days annually and one paid birthday per year (following one year of employment).Support for professional development, including continuance of salary and allowance.This is a newly established position to meet organizational needs.
