The New Vision Family Health Team (NVFHT)is seeking a full-time (0.8-1.0 FTE), permanent, Registered Nurse.

The NVFHT is a multi-disciplinary team located in the Waterloo Region providing primary care to over 30,000 patients. We offer a friendly, collaborative and professional environment in a spacious facility.

As a key member of the New Vision Family Health Team, the Registered Nurse (RN) provides comprehensive primary care nursing services with a strong focus on chronic disease management, particularly hypertension and diabetes. The RN works collaboratively with physicians, nurses, allied health professionals, and administrative staff, while also functioning autonomously within established medical directives and professional standards. Strong interpersonal and communication skills are essential to effectively engage patients, promote self-management, and optimize health outcomes.

Key Responsibilities :

• Participate in and support chronic disease management programs, with a primary focus on hypertension and diabetes care.
• Conduct comprehensive patient assessments, including vital signs, physical assessments, screening, and data collection.
• Provide ongoing monitoring, education, and follow-up for patients with hypertension and diabetes, supporting treatment plans and lifestyle modification.
• Maintain and manage INR monitoring systems in collaboration with physicians.
• Administer medications and injections (including allergy and other injections) in accordance with College of Nurses of Ontario (CNO) standards, clinic policies, and medical directives.
• Implement medical directives confidently and appropriately within RN scope of practice.
• Communicate test results to patients and coordinate follow-up care as directed by physicians.
• Triage and respond to patient telephone inquiries within scope of practice, escalating concerns as required.
