CTN’s Child and Family Wellness Program supports kids and youth with disabilities and developmental needs, and their families, through wellness-focused clinical services, program development, and network-wide capacity building. CTN is an Ontario provincial government funded not-for-profit organization that offers services through contracted partners in education, health and community sectors. The network uses an electronic client record shared across partner organizations to support integrated plans of care and services. What You Will Do :
CTN is looking for a permanent, full-time Child and Family Wellness, Program Coordinator who will identify and provide wellness-focused clinical services to kids and youth with disabilities and developmental needs and their families, and increase capacity within the Family Services team and the broader Network to support family wellness. This position contributes to the planning, development, delivery and continuous improvement of the Child and Family Wellness Program and reports to the Supervisor, Family Services.
Responsibilities :
Deliver wellness-focused clinical services through in-person and / or virtual group programming for kids or youth with disabilities and developmental needs, and their families (may include non-CTN sites or evenings as required)Develop / share wellness resources for CTN clients, families, employees and Network Partner AgenciesProvide opportunities for families to connect and share their experiences through clinical programmingExpand opportunities for shared learning and capacity building related to wellness within CTN and across the networkProvide CTN and Network support, collaboration, and capacity building related to wellnessBuild relationships with CTN clients, families, employees, Network Partner Agencies, and other external agenciesSupport and encourage family engagement and capacity building opportunitiesIdentify and assess risks to the client and family, initiate links to services and supports, and collaborate to determine appropriate actionsLiaise with external vendors as appropriate / assignedCorporate and Network Responsibilities :
Work closely with the CTN Leadership Team to support the work of the Network and communicate provider, family and volunteer feedback to the Supervisor, Family ServicesParticipate in and lead Network committees, task forces, work groups and other forums, as assignedParticipate in community worktables and service planning initiatives as assignedImplement CTN Policies and Procedures for local team operations, partner collaboration, asset management, health and safety and related operating requirements in collaboration with the Supervisor, Family ServicesWhat We Need :
A minimum of a three-year University Degree OR a three-year College diploma with professional designation in Social Work, or a relevant child and family focused discipline (e.g., Social Work, Nursing, Occupational Therapist, Physiotherapist)Must be a current member in good standing with the appropriate regulatory collegeA minimum of five (5) years of experience supporting kids and youth with disabilities or developmental needs and their families is requiredExcellent organizational, project management and interpersonal skills, including strong oral and written communication and presentation abilitiesSound professional judgement, tact and flexibility in interacting with a broad range of kids, families, and professionalsModerate to advanced technology skills, including creating PowerPoint presentations, Excel spreadsheets and other online tools including social mediaAdaptability to work within an evolving service delivery systemStrong organizational skills including time management, prioritization and project managementAbility to work independently and accurately with frequent interruptionsAbility to work flexible hours to support requirements beyond typical work hoursAbility to travel to assigned offices, between CTN sites and / or other locations as requiredKnowledge of children’s service systems in Simcoe County and York Region is an assetAbility to communicate in French or other languages is an assetExperience with diverse cultures and communities including Francophone, FNMI communities is an assetCertified First Aid is an assetDemonstrated commitment to a safe environment by working in compliance with the Occupational Health & Safety Act and related policiesSelected Successful Candidate Will Be Required To :
Produce a clear police record check with a vulnerable sector check prior to start of employmentBe able to travel to assigned office, between CTN sites and / or other locations as requiredMaintain the ability to work effectively in a temporary hybrid environment and have access to appropriate internet capacityProvide documentation to confirm eligibility to legally work in Canada (e.g., SIN, birth certificate, passport, citizenship, permanent residence, open work permit, or post-graduate work permit application receipt)Office Location : Richmond Hill, Ontario (location noted within posting)
Salary Range : $78,540 - $87,267
